NG i went back to the good doc i like to see and he gave me Valium, just told me to be careful, i got 2mg tabs and have to take one to two twice a day but try to only use it when i need it which i do today :( really sore so far. I took two about and hour ago and havent noticed much difference yet, can they take a while to kick in? This fissure seem immune to anything going!!

Hey there, glad to see he was at least willing to let you try it. For me Valium can take a while to kick in if I ate a meal before taking it, but even then I can feel effects within about 30-45 minutes. If I haven't eaten then I can feel it within 5 minutes or so. Usually I feel the mental effect of relaxation and the sphincter muscle doesn't feel like it wants to push as much anymore.
The ones I have are 5 mg each, the doc was originally going to give me the 10 mg ones but since I'd never tried it before, I told her I'd take it easy the first time around. I take 10 mg at a time if the spasms are real bad, but usually 5 mg or even 2.5 mg (I use a pill cutter to split the pill) is enough. The explicit instructions from my doc are to take 5-10 mg as needed for spasms, no closer than 6 hours apart. She suggested I try taking it a few minutes before I have a BM. For me though it seems that nifedipine ointment is better at loosening up the sphincter before a BM, and Valium is better at controlling the post-BM spasms.
I'm sorry to hear that it hasn't gotten you any relief... I think it will depend on what the root cause of your soreness is. If the soreness is due to the fissure being irritated by trauma like the passage of stool, the Valium won't help much. If it's due to the muscle being in spasm then I'd expect it to at least do something. When I first got back from my emergency room visit a few weeks ago, I was in terrible agony from very painful spasms (the intense kind that feel like they are shooting up your rectum) and took 10 mg of Valium (along with an opiate painkiller) and it truly gave me nearly instant relief. I was still sore but those muscles stayed in place pretty nicely afterward. I've been taking a low dose of Valium every day (usually 5 mg in the morning and 5 mg in the evening) since and haven't felt the same incredibly intense spasms since then. I don't know if I actually still need it but will be taking it until my CRS appointment next week when the doc can examine the fissure and tell me if anything's changed.
So anyway I guess it could be that the dosage isn't high enough, I suppose, as my doc told me to take 10 mg up to three times a day. Or it could just be that your fissure is worse off than mine. Following your doc's advice is probably the best bet, he knows your personal history a lot better than I do, so consider these just my personal experiences.
What procedure are you having done next week? Is it LIS?
