Anal fissure

Hello everyone. I’m a 29-year old male, who is currently struggling with a new case of AF. I first noticed that I got the AF after passing a hard stool, which hurt and after wiping, I noticed the blood streak on the toilet paper, which looked fresh. After 2 weeks, I went to the doctor and I was prescribed miralax. I took it for about 2 weeks and it seemed to have helped, especially with softening up my stools. I had been okay for about 4-5 days and then yesterday, I felt like my AF may have been reinjured. I don’t want this to become a chronic issue :(. Any suggestions would be greatly appreciated it. Sorry that you are dealing with a painful fissure right now. Don't lose hope! You can heal without this becoming chronic.

I would suggest that you find a colorectal specialist who can prescribe an ointment for healing like nifedipine or nitroglycerin. You're already doing the right thing by using miralax, perhaps you could add in a warm sitz bath as well. It could help to do this before a BM or after, whichever helps more with the pain. Avoid wiping with dry toilet paper, and instead clean up in the shower or pat dry with wet paper or use a portable bidet. If you can handle it, try massaging the area internally gently. There was a paper published where they found that massage was a big help in healing acute fissures. I don't have the link at the moment though.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

Miralax, sitz baths, and seeing a colorectal specialist are all really good ideas. Two more inexpensive and conservative measures that I can really vouch for are 1) Get a squatty potty (or find some other way to help you squat, some people just using books to prop up their legs) and 2) Applying a little vaseline to the area right before going can really help protect things. It sounds like you are catching it pretty early, so following conservative measures (plus of course diet and water) has a really good chance of clearing this up for you.
