So, I'm suspecting hemorrhoids have joined the party now, but I'm really confused about the pain. Has anyone had internal hemorrhoids that did not prolapse (so you can't see or feel anything on the outside), but caused you miserable pain that felt higher up inside the anal canal? Can such low-grade hemorrhoids that don't bleed and don't prolapse cause extreme pain inside?

I have absolutely zero bleeding, but this pain inside, on my right side, is just adding to the bum misery here. The pain is sharp like a knife (but not the way a fissure feels), and there is also this kind of a pressure like feeling too. Unlike the fissure sites there isn't really much, if any pain, during a BM. The pain sets in right afterward. Absolutely everything I read about internal hemorrhoids does not indicate any kind of internal pain like this. Usually stuff written about internal hemorrhoids indicates they don't cause any pain at all, but usually bleed, and when they do cause pain, it's more of an itchy, sore feeling further down toward the outside of the anal area. This has been going on for weeks, gosh, could it already be months at this point?. and with all my other troubles this has just sent the stress and anxiety through the roof.

So so frustrated at this point and have come to yet another end of another rope with these bum problems.

Secondly, I've heard mixed reviews on banding (if this is hemorrhoids), so when I see the doctor, if it is hemorrhoids, and if banding is offered I have no idea what to do. With a sphincter muscle as tight as mine is I'm concerned about how well it will heal as I know banding leaves a little wound in there. I'm also concerned about how much pain both the procedure and the recovery are going to cause. But on the other hand I'm absolutely miserable like this right now. The pain is not something that just lasts for a little while after a BM. It is there all the time and never lets up. (No indications at all that it is an abscess or anything. There is "nothing" there).

I had internal and external hemorrhoids and had to have them removed 10 years ago when I was 5 months pregnant yes mine got so bad that they had to put me to sleep and do surgery while I was pregnant. It is very painful for about 3-4 days following the surgery and gradually gets better day by day. Now I am going in Thursday because I have a anal fistula that I have to have taken care of.

Hi jr2.
I was diagnosed with low grade internal hemorrhoids (along with AF) that never bleed.
And I am experiencing pain higher in the rectum too. I was told that internal H should not cause pain, but the pain I have is different from fissure pain and I wonder too if it comes from hems. On the digital exam (twice) I felt doctors finger touched the hems, but it did not hurt at all. My pain upper from the anal canal is stronger when I am doing something (walk, clean the house) or when I eat something that blows my stomach. My fissure is about 1cm high in anal canal and is small, but I feel exactly the spot where it is. Besides this I can feel the inflamed and bruised upper end of the anal canal (dentate line) which is very painful and perhaps this inflammation is the cause of the pain upper in the rectum. I am actually in such a bad state that I can not do much and I must rest for almost whole day and even with that the pain is constant and is taking my life.
Sorry jr2 for your troubles. My post is not helpful, I know. I just try to tell about my similar pain and that I am wondering from time to time if it comes from hems too. I also read the answer from Jan (on the topic Me vs AF) which explains about it.

Disabling burning, drilling pain in anal area for almost a year, started after UT infection. Small anal fissure diagnosed, turned out it is probably not the cause of pain, MRI shows no inflammation or fistula. Grateful to forum members for support.

Thank you for jumping in with your reply. Sounds really similar for me. I pretty much can't do anything during the day but lie down. I can't sit, and moving around generally makes it worse. Sometimes a short walk doesn't hurt too much, but rarely.

If your doctors come up with any kind of new revelations for you, will you please let me know? (I 'll do the same).

I made a surgeon friend back in the early eighties at st marks hospital who i still keep in contact with and his current thinking when things are not that bad is to use preparation H

i use it all the time and it's certainly worth a try as a last attempt for a fix before any surgery is undertaken

Maybe is the fissure. When I had my fissure, in my way to healing. I stop bleeding and hurting during BMs. But instead I got this pain internally, something very close of what you describe, only after my BMs , like 10 or 20 minutes later and also this pressure inside. It was painful to walk, or run or bend over. Also I got this ache in my tailbone that was unbearable. I was convinced I had an internal hemorrhoid in addition to my fissure. But not. My CRS told me it was my fissure and that sharp pain and pressure I had after my BMs were typical behavior of a fissure, because internal hemorrhoids don't hurt. When I was able to cure my fissure all the pain went away.

A lot of discomfort in this region is a mixture of things going on like piles / fissure/ Proctitis an inflammation of the anus and unless you can see a surgeon ASAP which you cant on the NHS in the UK you invariably end up going round in circles as you need to pinpoint exactly on what's going on at the time when the pain is at it's worse

Proctitis particularly can be variable especially in men as men can also have non-bacterial prostatitis which leads to the rectum feeling sore or cut but is not hence the overall difficulty in the treatment of the bowel and as i have said unless you can get to see a surgeon as and when you can like me go for years getting nowhere and have to endure a life with mild discomfort punctuated with days when things are not so good

From my surgeons viewpoint this is the case for some in life like myself and there is no cure as everything looks and feels normal apart from some small internal haemorrhoids which is normal for half the population

Rio, directed me to your post & again I find another member whose pains sound oh so similar to mine.

I feel your frustration of not entirely knowing what is going on down there, I'm in the same boat! Internal hemorrhoids are the only conclusive diagnosis I have been given so far, although my GP felt granulation associated with a fissure attempting to heal at one stage yet a sigmoidoscopy not picking up a fissure but only a few internal hemorroids which are supposedly not meant to cause such pain. I'm lost with it all, so many different ailments present with the same symptoms that it forever seems to be a guessing game. :(

I hope you find some answers real soon & I will keep checking back on your progress & let you in on any further developments I may come across too!

Thank you all so much for your input. Went back to CRS. She did an anoscopy and said the same thing the other CRS said. Yes, there are internal hemorrhoids but no, she doesn't see how this could be the cause of my pain. But she also had no idea what to offer as an explanation either. I know it isn't fissure pain. I've had fissures for so many years I am really aware of the kind of pain I get from fissures. This is in a completely different location from any of my fissures so I know it isn't scarring from fissures.

So, next stop is a pain specialist, not just for this pain, but for chronic pain I suffer with in addition. Hopefully he'll have something to offer because the colorectal surgeons understandably don't know what to do with me. They are surgeons, and they aren't recommending surgery for me, so they've reached their limit with what they feel they can do. My GI doc is also at a loss.

So, we'll see what happens next...

Hello friends....I have had doctors say that the internal hems can't cause pain like this. My pain is excruciating, debilitating, and totally unbearable which has affected every aspect of my life since mid-October 2013. My pain is inside the rectum, down the tailbone, under the tailbone, and sometimes it hurts down my butt cheeks. BUT....one doctor I saw two weeks ago did tell me that the swelling and inflammation of the internal ones DOES cause severe anal spasm. So although the hemorrhoid itself isn't painful, the spasms caused by the swelling can be excruciating. Something FINALLY made sense to me! I am an RN and like all nurses, we need specific answers. I am still recovering but starting to get a little more hopeful that we can all get through this. I hope this helps.
