Hello everyone,
I am new to the forum and hope it can help me. To give you some background I started with on/off anal pain in 2007. I had it looked at with a sigmoidocscopy and nothing was found. Pain went away after a few months.

It came back in 2014. On/off anal pain. It then became more constant. By this time i was getting buttock pain/sciatica too all on left. The pain since then has always been left sided about 5pm location just inside anus which can spread to buttock at times. I got a referral - it was looked at, he found a small prolapse (which he fixed) and minor hems and no fissure but did a small stretch for good measure. None of it really helped in the long run. I have constipation issues and a borderline underactive thyroid. Not on meds for thyroid as GP says if I feel well ie not tired and I'm underweight weirdly not overweight then no need to go on meds for that. BUT I think it may be causing sluggish bowel.( which I've always had since teenager)

I use fibogel twice a day and eat well/drink well and exercise, but still have days of hard stools. This is what sets off pain cycle for me and this left hand 'patch' like a sore ulcer inside the anus. I can go a week or so with no pain at all, then bingo it's back. GP currently prescribed Rectogestic I started it last week but can only use a tiny amount 0.5cm or gives such bad headache. GP offered to refer me if this doesn't stop pain cycle. She couldn't feel/see a fissure but said it sounded like one was being opened up and closing again. There is no blood and never has been. I'm not sure it even is a fissure the last specialist did not find one!! I have had three children by the way and I'm 43!

Can anyone relate to this, I've been on this pattern since 2014 and every time I think it's gone it comes back maybe after a day, a week or even a month. Any help on hard stools or anyone in a similar situation and should I get referred would be amazing as I feel so alone in this and worried it will never go away.

I suggest you get on a regular stool softener like miralax, so that your body can get a chance to heal. Perhaps try Vaseline on the sore spot before a BM to see if that helps the pain. It's not something to use long term because the mucosa needs to breathe. Pelvic floor PT might be a good option too, in case the cause is muscle imbalance.

Hi patience and healing, thanks for the info. I will see if I can get miralax in the uk. Not sure where I start with pelvic floor PT here in uk but I think it’s an idea as there could well be something like that going on due to pain spreading into buttock. I will look into this as well.

I have had similar experiences to you but I did have a fissure. Mine apparently is healed but has left me with a great deal of scar tissue that is almost always uncomfortable. I too struggle with maintaining a consistently soft or manageable stool. Miralax just doesn’t agree with me but that’s just me. I take Milk of Magnesia every day but recently have found I have to take more to keep things moving. Life has been stressful lately and I really haven’t been able to exercise the way I would like because of always being uncomfortable. My diet is very good. Any other thoughts on keeping ones stool soft?

I forgot to add that I too have been referred for Pelvic Floor therapy as well. I recently saw a new doctor and she said there was nothing surgical she could do but felt it was all related to this pelvic floor issue. I also get pain in my left buttucks sometimes it moves around to other places, it’s strange.

Hi Fissure traveler, Wow you do sound similar to me., with left sided buttock pain etc.
I will be really interested to hear how you get on with Pelvic Floor therapy. Are you in the UK at all? I decided the Rectogesic hasn't made much difference, at least not yet so I went back to my surgery and asked to be referred to a specialist who will I expect do a colonoscopy. I had this before so I know it's not fun and was trying to avoid but it keep reoccuring. I at least now want reassurance it isn't cancer as my GP scared me into thinking it could be.

I was worried about milk of magnesia (I have used it) because of not really it very good for you long term and I need a long term solution. GP no help on this. My latest solution is a spoonful of olive oil on an empty stomach. I will see whether this helps. Trouble is I think everyone is different and some things work for some and not for others. If other people had my diet at the moment I think many would have very loose stools! But not me.
I would really like to hear how you are getting on with the therapy when you get started.

Love your comment about loose stools, I have been saying the same thing recently! Will keep you posted on the pelvic floor therapy- seems like everyone in my area is booked up until February. I live in the U.S. Stayed home from work today as I have been in and out of the bathroom multiple times and I still don’t feel empty, this has been going on all week.

It's very frustrating isn't it!!. I'm sorry to hear you are struggling today. I'm really interested to know if the therapy works and what it involves. Annoying to wait such a long time for it though. It's the same over here for anything with the NHS - my referral will take a while to come through.
Hope you feel a bit more back normal soon.
