Update:

I'm still mostly better, but having a bit of a set-back with some slightly painful mornings. I walk to work, about 1.5 miles, and recently I've been having some pain for the last 1 mile. Once I get to work and can sit down as well as move around, I'm fine for the rest of the day.

I think its because I got less good about drinking water, and over Thanksgiving I wasn't eating my lentils and quinoa. So, back on the wagon for me.

It's still a world of difference from the time I was walking at snail's pace. The walk took maybe twice as long because it hurt so much. Now I can go at my usual brisk pace even though there is some pain.

As an aside, while I was in pain most of the time, walking slowly all day long, reluctant to do much and getting little exercise, I did put on about 5 pounds. Once I was able to move freely and walk at my normal speed, the weight just dropped off. I was amazed at how much difference just normal activity makes to how much food I burn. The weight did add to my feelings of depression and hopelessness.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Sorry about the set back, but I am glad that you are still mostly better. Setbacks seem to be the nature of this affliction. Do you still have pain every day at some point? What regimen are you currently following? How many months are you into this now? Can you still see a time when you might be completely healed, or do you now think best case is a reduced pain intensity and less frequent overall discomfort level?

Bless you, PGA - you are always a source of encouragement!

Stillhoping, my whole journey began in May, and I finally stopped taking nitro in the middle of November.
I would say most days I have little or no pain. I get pain or discomfort if I either
1) don't pay enough attention to keeping my stools soft, or
2) overdo it by lifting something heavy, working too hard in the garden, etc.

My regimen at the moment is, porridge every morning with raisins and/or grated apple, sometimes almonds. Anything for lunch. I try to have something fissure friendly for dinner every day. Lentils or brown rice or quinoa. Lots of water all day.

If I'm worried I didn't eat the right things, I'll take 1 tablespoon of kondremul before bed. I use a hot water bottle under my bum at night, to relax the sphincter and get some good blood supply going. Ibuprofen if my bum hurts in the morning. No nitro or any other cream at the moment.

I really don't know if this will ever completely heal. If I have a big BM or slightly hard one, I get a sore bum! Will this ever go away? I don't know. I told myself to give it a year, because I noticed that any injury takes much longer to heal now I'm middle aged (50). This injury is in a place of daily trauma so of course it takes a long time. Also, I might have a naturally high anal resting pressure, which will slow healing a lot and put me at risk of a re-tear. So I'll have to be careful probably the rest of my life. But I'm not a naturally careful person! I tend to over-estimate what I can do, which gets me in trouble.

I don't know if that helps you at all, Stillhoping. I'd say your chances of complete healing depend on so many factors, including age, anal resting pressure, how long you've had the fissure, what treatment you get, and so on.
When I feel a bit despondent, I remind myself of what it was like 5 months ago!

I don't dare lift any thing too heavy at work, I get some one else to help me, you should too. Good for you keep babying your self..chin up, your doing greatt. You will heal, in time, I felt the same, and I am healed, give it time my friend.

Your half way there..trust me. Sometimes having a larger B.m is OK as my Doc as confirmed, it helps stretch you...just give it time.

I'm still in pain - this is quite a bad set-back.
Yesterday I had pain/discomfort all day, for the first time in weeks.
I blame the calcium-fortified orange juice I've been drinking in the mornings. At my age I worry about getting enough calcium. The pills make me constipated, so I thought drinking calcium in OJ would be better. Unfortunately it seems it's just as bad. For several days I've felt as though I needed to "go" all afternoon and evening, but nothing would happen. Obviously I'm not going to strain, so the poo would stay inside until morning. I've been taking kondremul in the evening so the morning poo wasn't too difficult, but I guess it was dry enough to cause problems.

Well, at least I know what to do. Pour all that calcium orange juice away, sit on hot water bottles, up the kondremul for now, take ibuprofen, eat my fissure diet religiously and drink loads of water. Unfortunately I've had to start using the nitroglycerine again. Hoped I'd seen the last of that. Luckily I kept it just in case! The first time I used it I got quite a throbbing in my head! It soon eased off though.

Hi Scientist2516, sorry about the setback:

Please don't let this qualm your spirit too much now, ok? I'm sure you'll
be back on track soon. Yes, it's quite natural to be concerned about your
calcium levels especially consuming CCB's.

Perhaps a blood test is in order somewhere down the line to check the
levels, especially if osteoporosis or the likes runs in your family.

But for now, please stay true to that routine that brought you this far
dear. As the holidays are beckoning we sure hope the discomfort down
there is not too distracting.
