Msimon,

My pt told me that my burning is my muscles spasming because they are tight...
2/14 Fissure developed
3/14 Diagnosed w/ fissure given Nifedipine
4/14 Referred to Pelvic Floor Physical Therapy=Pelvic Floor Dysfunction
5/14 Fissure declared "healed"/chronic anal pain persists
9/5/14 Botox to pelvic floor
9/22 biofeedback

My fissure is slightly left of posterior but pain is higher up on left side. Gets progressively more uncomfortable the longer I sit down. It is like feeling hot poker sticking in there.

At its worst my whole bum felt like it was on fire and heating pad revved it up but hot bath usually soothed it. I used to sit on bag of frozen peas to help with pain. I had internal & extern hemmies as well though so my pain was also to do with them

I have dull, sore and burning pain too. and that dull burning feeling goes all the way to my lower back. Call me crazy but- I was running for couple of months until my knee started to hurt. Went to physiotherapist and he said that the pain comes from really really tight back muscles, which are straining the muscles in thighs and respectively, my knee. I am personally sure its because of AF. I am never relaxed, those muscles down there in back are always tense. Going to massage soon, so hope it will help...
Lately I started taking Ibuprofen after my BM

Mine is more on the left side, but the pain and pressure builds up through the day, then it feels like pins and needles in my bum and feels like I need to go when I don't! Truly awful. The spasms are worse for me as I often don't have pain on BM when my fissures aren't acting up. Aghhhh

It helps me to take ibuprofen as soon as I wake up, so it can taken effect before my BM.

I had what felt like a burning pain in my rectum which lasted for a good 6 hours after I'd had a bowel movement. Sometimes it would be worse than that and I'd get this feeling like I had a tennis ball up there, very uncomfortable to sit down. I developed a knack of "sitting" on the sofa with my feet on a footstool and my bottom hanging off the edge. When they gave me all these products that needed applying up my bottom, I found it impossible to do. I have to say though, I had diltiazem ointment which did help eventually.

I also took voltarol (diclofenac) 50mg.

Yes . . . .and yes.

When I am spasming, I use my diltiazem ointment and a gloved finger, and patiently press the ointment into the hard spasming area, while remembering to breathe (instead of clenching and/or screaming) to try to relax. When I feel it begin to "soften' and give, I press gently but firmly until I can insert the finger. Then I work it until it is open. On a bad day I might have to do this rather frequently, say, every couple of hours or so. At the first sign I take a vicodin (pain) and a dose of medical marijuana (for the relaxation) and sometimes a Bentyl for the spasming.

And yes, I have trouble getting my bladder to let go when my sphincter is in a hard spasm.

Most people in my life can hardly understand the pain from fissure, let alone how very disruptive and unexpected the spasms can be. It can be very disabling.

Yes, I take ibuprofen on waking at the moment too.
I have had a wide range of fissure pain over the years .... From the knife like stab, to the sitting on a golf ball, to the dull achy throb. All this pain from one tiny cut!!
