Hi, I am new here and new to any forum type thing but I am in so much pain I need help. I am 36 and for the last few weeks had some pain which I assumed was piles. I used cream but the pain got worse but was relieved after movement or walking. I went to one dr who gave me rectogesic.... She did not tell me she thought I had a tear (although she put this on her notes). The cream gave me headaches so I stopped using it. I went back a few weeks later and was given Proctecdeyl which didn't help. Then after a few days the pain was omg.... I needed baths, creams and I tried everything. Last few days have been good as I have gone back to rectogeic as I was told by my gp I possibly have a fissure as piles should not be that painful. I am on stool softeners but today I had a 3rd poo and my god the pain has been horrific for hours. I'm literally lying in pain...... Is this normal ??????? The pain is the worst ever...... I am seeing a specialist Saturday private as I can't take the pain. My question is should the pain be this BAD ??????

Yes, sadly, the pain can be bad - really bad. With an active fissure, the pain is exactly as you describe. It comes and goes. The BM itself is often painful, and then, sometimes 30 mins-1 hour afterwards, the after-spasms start, and they can be agonizing.

I'm sorry. We all know what it's like, and you are in good company here.

I will say that if rectogesic is helping you, that is excellent news. If you can't stand the headache, ask for nifedipine or diltiazem, as they tend not to give headaches. Some people don't find them as effective, but you may be one of the lucky ones. Still, if you can keep using rectogesic, do keep on it, and this thing may heal without surgery. You want to cure this as quickly as possible so it doesn't get chronic.

The pain won't last for ever - it will gradually improve. I know it's intolerable right now, but hang in there and keep on using your stool softeners and medication - it will get better. You'll notice an improvement even before it heals completely, so endure for now, and keep posting on the forum so we know how you are getting on. Ask any questions you want to. We are here to help, and we know what you are going through.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Hi thanks so much for quick reply its great to see that I am not alone. The pain today was bordering going to A&E but has finally subsided a bit. I can honestly say pain wise it's up there (I had a kidney stone once and this isn't far off) I don't want surgery due to work although they have been great and I am on restricted duties and less hours. All the creams were rubbish. I have tried all the witch hazel but nothing touches the pain when it's really throbbing. GP has been sympathetic but said there is nothing more they can do hence why I am seeing a specialist Saturday private............ My fissure is hard to see and touch and is a little deeper I think. The dr done an examination and it was fine until she moved her finger and jumped out of that bed............ Anyway all the advice is well received and I will continue even if mine heal to help if I can in anyway

Sorry you are in so much pain. Not that it is much comfort bit it is typical. I'm glad you are seeing a specialist soon, its important to get the right treatment early so the fissure is not chronic. In the mean time keep you BMS soft, if you can take warm baths after BMS and if you have any pain throughout and can take more baths do so. Some people like to sit on a heating pad, but that didn't help me. Hopefully the Rectogeic will work for you but if not discuss what the next steps would be. Good luck

For some reason I posted my password. Thank you so much for reply also apes. I have never been on forums or the internet as I leave that to the kids but I was in so much pain I started looking online and saw this and had to post something, Its nice to see im not alone. My wife hasn't really been sympathetic until she saw me crying today (a grown man crying freaked her out) The pain has eased now and im looking forward to a good nights sleep but im dreading tomorrow :(((

You're right about the pain being the worst ever - I'd take natural childbirth any day over this. Some other things that helped me during the initial stage: putting my feet on a stool in the bathroom (eventually switched to a squatty potty), taking ibuprofen as soon as I woke up so it would take effect before my BM, using the finger support method, and using a spray bottle (portable bidet) to clean afterwards. The finger support method is where you press halfway between your tailbone and rectum during a BM. It's very helpful for posterior fissures.

By the way, don't expect the rectogesic to give short term pain relief. It's more of a long-term thing. It relaxes the anal sphincter and dilates blood vessels to allow more blood supply (=healing) to the area. It certainly helped me, so persevere for a while, anyway.

Hi and welcome. You have come to the right place. We all understand what you are going through. The early days are the hardest I think in part because there is so much confusion if you are not familiar with fissures. There is so much good information here to help. Soak in very warm water after each BM and whenever the pain is bad. It helps so much. In the beginning I was in the tub 3 times a day. Keep stools soft. I have found Miralax to be a life saver. Drink a lot of water. Keep to healthy foods only. It won't be long til you see the specialist. So sorry you are suffering.

Thank you everyone for you advice and support.... The rectogesic has been on for a bout a week and I thought things were great until yesterday when I had a third number 2 and that's the worst pain ever..... I take it that is normal for the odd return during treatment. I am pain free this morning but dreading the loo however I won't hold it.

Been two hours after big but loose BM pain is actually not as bad as yesterday and is ok to put up with. I hope this means it's healing...... Wanted to take family out for a meal but don't want to jump before I can walk..... Thanks again everyone
