Healed fissure but still have a tight internal sphincter

Postby Alyssa » 18 Oct 2014, 22:29

Does anyone have a healed fissure but still have a tight internal sphincter? Botox was placed on my pelvic floor muscles to relax the muscles but my internal sphincter is still tight, when stool passes too quickly it stings and leaves me sore; when I sit I get this hot burning pain which my doctor says its the nerves that are being compressed. Today I massaged the area, and I noticed that my sore spot is at the internal sphincter... When I touch the (band) it's painful!

I will be getting Botox into the IS in December... I have also requested a pressure test just waiting for colon rectal surgeon to approve it.

my chronic pain doctor told me that she did not need a test to determine it but i told her that i would like one because i've been seen by several doctors which have said different things (I have a normal sphincter/ tight sphincter) so I wanted this test to determine it once and for all.

Alyssa, I am sorry to hear you continue to have so much pain. I too have pain when pushing on the internal sphincter. In fact, I can insert a tiny applicator in there right now :( I just wanted to mention that I think the reason why people get such varying observations when their internal sphincter is palpated is that the thing is always in a state of flux. It tightens when we are nervous, after a bowel movement, during stress, when there is an injury, etc. It is relaxed when we feel rectal fullness, calm, etc. So getting a pressure reading or a Doctor assessing things will only tell you what it is like at that moment in time...

As a patient the different diagnosis have been frustrating... Now that my pelvic floor muscles are nice and relaxed, my doctor is able to determine that my internal sphincter is tight.

When my stool sits there (IS holding it in), I feel like the stool irritates/ burns the area... I used to feel papercuts before I got Botox, now just soreness...and burning pain. Weird sensations...

I'm in exactly the same position!! My fissure is healed but my sphincter is sore to the touch in specific parts, I had sclerotherapy months ago which pretty much healed my hemorrhoids but I'm just left with this horrid pain!

when i have a BM i try to shower off but the jet of water is like daggers on two specific parts of my sphincter. do you get the same?

it's just horrible :(
Suffering since June 2013

Methods tried:
GTN cream - migraines!
Manuka Honey / Coconut oil / Aloe
tag excision X 2, botox x 3
Dilatiazem
dilatation 2 X daily + Retin-C Vitamin Scar Treatment Oil 2 X daily - best thing ever.
Now pain free 2/7/21

Yes, I do have similar symptoms.. Did you feel the paper cut sensations like when the stool exits it creates little cuts? I also get little stingers!!

Maybe you still have small fissures that are going undetected??

I shower off after a bm, and it feels like daggers on the wound from the flap surgery.... That's what makes me think you may have tiny fissures?? Just an idea.

My chronic specialist said that this is because I have tight muscles... Once I got Botox injected to the pelvic floor, now have soreness and burning...

I think you should see a pelvic floor physical therapist and ask for a manometry...
