Is my fissure chronic or acute?

I had a baby two months ago and I've had a sore bum ever since. They kept diagnosing hemorrhoids and treating me for that, and it got worse and worse until one day a few weeks ago I suddenly started getting those dreaded spasms and was in agony for half the day following a BM. I finally got to see a specialist and he diagnosed a fissure. He gave me some cream to apply four times a day and within four days I was pain-free. However, today a week later, I had a BM and it hurt a bit and is now irritated, though not nearly as bad as it was. Could it still be healing or is it getting worse again? I"m so scared it will go back to the way it was, which was shaking/screaming/crying pain. Do you think this is an acute fissure still healing or is it turning into a chronic one?

It might still be in the acute phase. Now is your chance to heal it. Make sure to get a stool softener, like Docusate. This will prevent the fissure from re-tearing from a hard stool. You need to be consistent and keep taking the stool softeners until the fissure has COMPLETELY healed. Also, if you can take a sits bath 2 times a day for 20 min, that'll accelerate the healing process. Trust me, I let mine get worse, and I wish I hadn't. It's much harder to deal with now.

Less than two months means it's probably still acute. You have a really good chance of healing without surgery, especially given you've already had a massive decrease in pain. Just know that healing a fissure can be quite the roller coaster ride with good days and bad days. Also, even when the worst pain is over, you can have low level pain for a while. I'm not telling you this to scare you. It's important to know that this is a normal part of healing. It can be terribly depressing when you have pain and you think you are back to square one, but you are NOT back to square one - it's just part of the healing journey.
Mary is right - do everything you can to keep stools soft. Take Miralax, docusate, or magnesium citrate - whatever works for you. And drink plenty of water to hydrate the stool.

Thanks for the replies! That makes me feel a little better. I could handle the pain I'm feeling right now forever, it's just the anxiety of wondering if the next BM is going to be the one that starts the nightmare all over again, that I"m having a hard time with. It was the worst pain I've ever experienced, including squeezing out an 8 lb 12 oz baby sans epidural. Hopefully it's just a minor setback!

Hi daisy!
I'm curious, do you have a lump on your anus which I've read would be a sentinel pile? I have an old skin tag but I swear I either got a new one or this is my old one just irritated. When I went to a CRS, the pain was so bad he couldn't look inside my bum to see if there was a tear. He had to assume from my amount of pain and discomfort. I mistook my bloody stools for hemmrhoids therefore I put off getting help thinking I could do it on my own. I also just had a baby and this all happened shortly after. I'm going on 4 hours since my last BM and I'm still feeling the sharp pain. I can't move. I lay down most of the day feeling depressed and hungry for all the things I can no longer eat. Doctor gave me nifedipine and lidocaine. I had a couple days where I just felt sore, but after a bowel movement I just feel like I'm back to square one :'( . I'm looking for all the support I can get. Thanks

I don't think I have a lump. I'm now on day four of no pain at all, so maybe I'm one of the lucky ones? For a while I would relapse every time I had an even slightly firm BM, but I've managed to keep them soft using Restorolax once a day, 1/4 cup prune juice in the morning and at bed time, oatmeal in the morning, sweet potato with skin every day, papaya every day, several prunes throughout the day. Things were working a little too well so I cut back the Restorolax to once every two days and that seems to be working. I'm not sure how long I have to keep this up, it's exhausting trying to maintain the perfect diet for soft BM's, but I'm scared to stop what I'm doing for fear that I'll have a relapse.
