6 year old with chronic fissure

Hi,
I hope all of you experts can help! My 6 year old son has had a fissure on and off for over a year now. Started with very small amounts of blood on tp and never complains of pain. Dr. recommended Miralax which took me forever to get the right dose for him. I have to do about 1/2 tsp every other to every two days or else he has accidents because it's so soft he doesn't even get the urge. Worked for awhile and so then started weaning him off, and then bleeding again. He has a BM about 5 days a week and when not on Miralax they are huge. Over the last month or so the bleeding is more as in it drips in the toilet water. Twice now he has actually bled a bit when just urinating. I've noticed when he urinates he pushes as if he's having a BM. I've encouraged him to relax and that he doesn't need to push it out. I'm assuming that the pushing from urinating and pooping is constantly opening the fissure. All the dr is saying is keep pushing Miralax to keep the poop soft. I'm also giving him a probiotic and starting a fiber supplement today. I'd like to get away from the Miralax if possible as he seems so sensitive to it and it's so hard to control the dose where he doesn't have leaks. The other day I picked him up from Kindergarten and he had a bit in his undies and he said he didn't feel it : ( His BM also seems very pasty and gooey on the miralax.

I have follow up with Dr. in 2 weeks and wondering if there are any other suggestions anyone has. Thanks!

Hi AFmom! I feel for your little one, and for you. As a mom we want to do everything to help our little ones be healthy, so I can't even imagine what you and he are going through. I had my CAF for close to a year. Just had LIS over a week ago (with complications of an infection and abscess). Other than offering you support I don't have much else to add. Has the doctor prescribed nifedipine? In a cream it was the only thing that helped me heal (I would suffer set backs every month or so). But that was the only cream that seemed to help. Has he tried the sitz bath as well? That helps with the discomfort after a BM, and helps clean the area as well. Is he seeing a specialist or just his pediatrician? Anyway, I hope you find more advice to follow, and that he starts to heal soon.

Thanks you for the reply. Luckily he says it doesn't hurt. His ped just thinks he's so used to it but he seriously has never complained other than saying he's a little itchy now and then. I will ask about the cream you mentioned at his next follow up.

At least it doesn't hurt. The cream may help him heal. Also the sitz bath helps with the itching. Best of luck to you and your little one.
