What came first

Im curious as to what comes first-Is it the anal fissure that causes the anal canal to spasm or the spasm that then causes a fissure or is it different for people.

Im sure with me it was a spasm first that stopped me going which then made me push?

It is not the 'anal canal' that spasms. It is the internal sphincter, a muscle that you cannot control.
