Dear over10years your story moves me a lot. Its good to feel I'm not alone in this and I know its a living hell. My suggestion is to get a good surgeon and have the operation done.
I myself had six of them 5 unsuccessful - with wound reopening over and over again which crippled my nervous system got me a hernia because of all the stress and put me in a living hell of constant pain, anxiety and panic attacks. My close ones even considered putting me in a mental institution for a while because as a result of all the pain I was acting suicidal. All this during time in my life when I became a father after 3 years of trying and finally a successful in vitro.
I also lost my job over this and thank god for my loving and caring wife who got me so far - because without her I honestly do believe I wouldn`t be writing this to you now.
I had my last operation 2 and a half months ago. Im still in tons of pain but it gets easier at times and this time wound didn't reopen one month post op. So Im keeping the hope it will get better so that I'm finally pain free one day.

Oh dear, that sounds awfull and a lot more painful than what I've been going through. Up until this week it's not stopped me doing anything major. Yes I lose time at home when I should be working etc but in terms of going to work or my leisure stuff I've always managed to keep going even when in severe pain. This week was different though and I decided to seek proper help for the first time.

I really hope your recovery from the last surgery continues to go well and that you are soon as right as rain. So pleased to hear your wife is able to be there for you, it makes a massive difference!! One of the horrible things about this pain is not being able to let anyone know how much pain you are in. I've been stood at work several times because I can't sit down and blamed the standing on a bad back.

Just a little update. With regard to the headaches from rectogesic, last night and this morning I managed a full dose without too much trouble. I get a very mild headache, more of a feeling than a pain, and also go a bit 'fuzzy' in my head but that's it. I've been taking painkillers half hour before applying and this morning felt confident enough to take just 1 insead of 2. All was fine. However I do apply it before going to bed and then the set my alarm so I can apply the morning dose allowing an hour an half to recover in case I can't drive and then go back to bed for half hour. So this means I always fall asleep And can't tell if I have a bad headache.

Now I'm just waitin for the stuff itself to kick in. Everything is still in just as much spasm, so still very painful after BM. Luckily I have gaps I between where the spasm although still there isn't bad enough to cause great pain. I'm just living in fear of BM now though as I get 12-24 hours of spasm pain afterward :(

GTN (Rectogesic and Rectiv) are proven to work through randomized studies for acute fissures, and anything that avoids surgery is worth trying. It takes using it for 6-8 weeks to say whether it works or not. Also, do not escalate the dose as that will not make healing improve. Just go with your doctor's recommendation.

I'm coping really well with the rectogesic now. The side effects have certainly tailed off to almost nothing. I don't need any painkillers and don't need to set my alarm to take it early, I can use it and go straight about my business. All I get is a slight inkling that a headache might start, but it doesn't, along with a bit of a fussy head for 10 mins, which again, is barely noticeable.

I think it's too early to tell yet whether it's helping (I'm on day 10) because I usually find after a flair up things will settle down around this time anyway. However I do get the feeling that the fissure itself has become less sore slightly quicker than it usually would do. I am hoping this is due to improved blood supply and therefore improved healing. It's only very slight though so could just be a coincidence. I'm not holding my breath just yet!

I managed yesterday and today a BM that wasn't too, too painful, which bearing where I was with it last week is amazing. Slight spasm yesterday, and today thought I wasn't going to have any but sitting made it start to come on. My partner very kindly bought me a cushion thing to sit on with a ring cut out so as to relieve the pressure and when I sat on that the spasm then eased away. I'm preying this could be the beginning of some sort of recovery without too many a set back along the way.

Have you tried halving the dose to start with? That's what I did and it helped. It took nearly a week (wish I'd kept a diary) to get up to a full dose. Also use a bit of cling film on your finger, I found a doc online that says this avoids absorbing more through your nail bed and helps to minimise the headache.

Over10years you said above you`re not sure you have one or more fissures. Why don`t you go to a good gastroenterologist and have a colonoscopy to determine the exact roots of the pain - one or more fissures if more - how many, and to rule rule out abscess, fistula, bowel disease, Crohn's disease or cancer.
Only then will you know for sure on whats the best way to proceed.
Can you pls also keep us informed on your Rectogesic treatment and if it has any effects?

Glad to hear that you are seeking medical advice for this. The GTN (Retogesic, etc) does work on increasing the blood flow to the area, causing the involuntary internal sphincter - which is what gives us all the pain - to relax, so to speak.

I saw different doctors (CRSs and GIs) and was prescribed both Diltiazem and Nifedipine. GTN too. No surgery, but different examinations/tests. In the end (ah the puns!), what worked for me, in consultation with my CRS, was dilation, which took a fair amount of time but eventually stopped the dreaded excruiating spasms.

Keep going with the Rectogesic! :smilyhug:

SurvivorX, I have a follow up appointment on Friday, so I'll see what they say then. Things have improved a fair bit in the the last 4 or 5 days. I tend to find that happens anyway after a flair up, so whether it's the rectogesic or just natural healing that's no different to before meaning I'll just get yet another flair up (each time it seems to get worse than before) I don't know yet. I guess only time will tell, and also whether I can heal enough before becoming unstuck with an uncomfortable BM that undoes all the healing so far.

Yes I will definitely keep everyone informed as to how things are going.

Thanks for the post Savaici. Can I ask for a little more info on the dilation you did? I see from your sig that it was self dilation, was this under the guidance of a specialist or GP? Were you provided with the tools for this/were they medical, or did you buy them off the internet. Also how often and for how long each time did you dilate for? Etc, etc. This is something I would like to try and am going to ask about it.

Long (and personal) story, but it's not my GP I've seen for help with this. However I am seeing my GP tomorrow (Thursday) for something else, then have my follow up for my fissure/s on Friday. So will be filling my GP in on the details and what they've prescribed etc. I will probably ask about dilation, but I don't know if she'll want to leave things to people who are treating me.
