Aching/stinging pain in perineum

Hello everyone,

2.5 years ago I had a perianal abscess/fistula; it all healed fine after the fistulotomy and I had no further issues with it specifically. Then about a month ago I had a particularly hard bowel movement which created a small fissure in the scar tissue. It has also healed thanks to fiber and some medicated ointment. But there is one problem that has been plaguing me for the past 2 years...basically ever since the fistulotomy healed. I get frequent feelings of stinging and aching in my perineum. I can only feel it when I touch the region, so it isn't something that bothers me constantly. It does, however, have me puzzled. If it were on the fistulotomy scar specifically I would understand and not be surprised, but this stinging/aching occurs on the opposite side of the perineum as well, where there never was an abscess or fistula. It is almost exclusively limited to the "taint" area between the testicles and my rectum and also on both front side of the perineum (but nothing in the middle or back of the butt). Do you guys have any idea what could be causing this? Its been going on for years (varying in intensity). Is it possible that the fistula surgery messed with the nerves and is causing problems all over the area, even in areas that weren't cut? Or is it something as simple as me rocking back and forth on the toilet wiping way more often than I used to? Does anyone have any similar pain? I've pressed hard on all of these areas just to make sure there is no mass or anything like that and it all feels normal with the exception of the stinging. It seems very tender to the touch (stings easily)...almost like a "hair pulling" feeling.

It could very well be nerve pain. Nerves can give off strange symptoms when they are cut and trying to heal. Maybe switch to using a bidet instead of toilet paper? Or you could try wetting the paper and wiping gently. A dermatologist might be helpful in figuring out whether it's a skin issue that you're suffering from.

9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

Hey Cporosus1- how are you doing? I have been following along with your healing process. I am at almost 12 weeks post op fistulotomy and I was doing pretty well in beginning, but ever since around 7 weeks the internal opening has felt more irritated and zingy. CRS has said all was good at at 10 weeks said skin was covering and dismissed me. I had had a previous fistulotomy over 20 years ago so scar always there giving me issues and now this new surgery. I did ok for a few days after seeing him last, but been doing a lot of walking and steps and had some food poisoning so D and now I have that tender, zingy feeling again- a zingy spot. He suggested I might always feel it- deep scar- also a little muscle exposed. But like you - if I feel something is not right I panic. I am going out of country in September- I guess if not better in a few I will go back in to see him- though frankly his communication and bedside manner unnerve me. Do you still have any aches or twinges? Does this sound in ra
nge of "normal"?
