Never Ending Fissure Saga

I have been suffering from fissures since 5 years. Its effected me personally and professionally. Like many others long term plans are on hold and it would appear they will remain so for the next decade. I am 37 year old Male and had my first fissure 5 years ago and it healed in 20 days, fast forward 3 happy years, I had another fissure when i lost my father. I think I was in depression and wasn't eating well, and then boom fissure is back. This time with a vengeance. It hides for a while, then its back. I have had 11 setbacks in the last 2 years and i don't even know what is going down there anymore.

Anyway to the current string of specific problems. I have a lump from the original fissure 5 years ago and it remained so. I have been taking miralax since 2 months. A sachet of 34g a day. The stools are real soft and shouldn't hurt ideally. To my surprise the last 2 times i saw blood and in the process re-torn it, the stools that day were very soft but had cracks in it. The cracks have done something to the lump or around it as i developed swelling around my anus and it was so bad for days that i cannot sit, sleep or move. They got fixed on their own without any ointment or me applying any thing. But it was still so soft that i did not think anything would happen. 2 years ago the same sort of soft crack consistency did not do anything but now even the most softest stool with cracks is tearing something apart and is impacting me. Imagine what would be the case if i had to stop miralax...its gonna be mayhem down there

What I believe now is the skin down there has become so fragile that it would tear to the most gentlest deviation in stool consistency. I am worried that this will never end. Has any of the long time sufferers experienced this. What are your thoughts to getting back to being sane. There is no end in sight. I cannot live like this forever. What are my options now given the state of fragile skin condition down there.

Also how long can one take miralax for. Google says long term use can bring about electrolyte imbalance and it can impact absorption of certain minerals etc........has any one had severe issues with long term use of miralax.

Any appointments to the doctor in my country are very strange. I tell them my story and the pain, the response i get is you should see the life of a cancer patient, that you are much better off. Unfortunately i live in a country where the pain quotient is high and people tend to get through to a lot of pain without even consulting doctors so the Doctors response doesn't surprise me but then again there is no attempt from any doctor to empathize and give me alternate options or some sort of hope. I am resorting to google and this god send website to vent it all out and learn from fellow sufferers.

:cry: I'm so sorry to hear your story! I can empathize with you as far as the frustration with retears. It's the same questions that I find myself asking each time... "why did this happen?! What caused this?!" :sadd: :wtf:
I've delt with multiple fissures for almost 3 years. My very first one was the worst! I couldn't move my body. I'd be stuck laying down somewhere for 8+ hours. Retears hurt but not nearly as bad. I can never tell if my same fissure is retearing. Do you suffer from a lot of internal hemorrhoids? I do.
I've noticed when the retears happen, the stress doesn't help, but it's the bad diet that I think really causes it.
Eating poorly can cause your body to inflame therefore causing your hemorrhoids to act up. Whenever I have bad hemorrhoids, I tend to notice the skin feels very fragile. I take miralax often. In the beginning I took it everyday for a whole year. Have you tried using coconut oil? Zinc oxide? As far as feeling comfort on this website, YES I feel 100% the same! This forum is the only place where other people understand where you're coming from, and how truly debilitating these conditions can be sometimes. :gah:

Thank you Sufferingbadly for taking time out to respond.

I haven't been quite myself the past couple of days with the pain during and radiating pain after bowel movement.

The doctor who examined me last time told me i had grade 2 hemorrhoids. This was 9 months ago. They could have flared up.

I dont have any reference on hemorrhoid pain. Does it hurt when stool passes through a hemorrhoid just like fissure. I currently have pain each time stool passes through.......and in terms of degree i feel its the same as fissure i had last time around. Does it sound like a hemorrhoid to you. How are hemorrhoid differ from fissure in terms of pain and other features. Can we tell from any factors or is digital examination the only way here.

Do you still stake Miralax. At the moment i am on it since 50 days and worried that i may get used to it and cause further complications. Is Miralax recommended for long term use ?

You know i can never ask these questions around the house, i feel like an alien as everybody around can see me in pain but i don't think they quite understand the emotional element here. They still think its a tear just like anything you see happen on any part of the body. I understand they cannot comprehend but it makes be all alone in this vicious cycle, more so emotionally. I guess we have got to learn to deal with it. The saddest part here is its gonna happen till the end of your time and all you can do is get in a zone and live in it which is easier said than done. Its like you are re-born after fissure and a totally different person now with a different perspective and attitude towards life. Gone are the days when you try and push the envelope in everything you do, now its all about being cautious and stick to basis forever. Makes you feel is this worth living for anymore!

Hi there, miralax is safe for long term use. Don't cause yourself more stress by worrying about being dependent on it. You're not abusing the laxative, just taking it in recommended amounts. It doesn't stimulate the colon walls so it won't cause dependency. It simply draws more water into the colon, so it's important to be hydrated while using it.

Internal hemorrhoids generally don't cause pain. If you have pain with and after BMs, it's usually a fissure that's the cause. Is there anyway you can find a specialist to help you with this?
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
