For me L glutamine caused constipation, but it may cause different reactions in you. I would suggest not trying all the things at once because then you can't narrow down the reason for something going wrong. Also L glutamine is not a regulated substance so it's best to start at very small doses like 1/4 tsp or so and see how it goes.
8/16-12/16: Fissure due to antibiotics

9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

I would be careful how much you add in at once, magnesium will cause most people who do not have a naturally slow transit to have diarrhoea so it is possibly that. You should always start slow with magnesium as our diets now....unless you eat organic from an established farm...are so deficient in it that it can be a bit of a shock to the digestive tract.

Saw my CRS today. After an exam he said he feels where it is tender but doesn’t see anything alarming. He gave me pain meds. He does seem concerned about my pain... he wants me to get a MRI. Although he’s not sure what it will show. I’m hesitant because with my insurance it will cost about $1,000. And that’s a lot of $$$ to show nothing.
Has anyone had a mri and has it shown anything?

An MRI is usually used to show up fistula or peri anal crohns. I know I had one to check for peri anal crohns...I’m not sure if you can check for fissures but it’s possible you can.
Yes it is quite a lot of money if you have no signs of either of those, maybe it’s the next step they take after everything else.

I also had an MRI scan specifically looking for fissures.

A little update. My kids have been sick and we were on vacation so it’s been a whirlwind here.
I have cut out most dairy, only thing I’m using is cream.( I tried cutting out coffee and got a massive headache.)
I have gotten gluten free wraps (yum....) and gluten free snacks so I’m pretty much Gf. And vegan cheese -
I have to say on vacation last week i felt better than I do this week. Pain is always there, but it didn’t flare up at all. And my BM weren’t always ‘smooth’ so bizarre I wasn’t in too much pain.
Now being home, pain strikes at night for hours.
Finally scheduled the MRI just going to bite the bullet and pay for it.

Since you felt better on vacation perhaps stress and resulting muscle tension have something to do with it? I don't mean to dismiss your pain as purely stress related the way doctors might do when they can't figure out the cause for pain.

I’m not sure but that’s a good point. The pain was still there... and for some reason before we left I knew I was going to be in more pain when we got home. I really don’t know what it is. My BM were actually bulkier there.. tmi... but I wasn’t in the pain that I am now.
It feels like something is pressing up aginsf my rectum like pressure. I took a tramadol which helps a little bit.

You could try some kind of barrier ointment during BMs like Vaseline. I've found it extremely helpful in the past couple of weeks at shielding a mysteriously sensitive spot higher up on the posterior side of the anal canal. My CRS didn't like the idea of Vaseline but I've found it helps me out for now.

So I got a MrI last week. I haven’t spoken to my doctor yet, but I got the results. Some of the terminology is confusing, but it says
simple linear intersphincteric fistula
has anyone had one? I have. And had a fistulatomy 3 years ago...
But I’m wondering if this is what is causing all the pain.
