Wow those vaginal ones look a bit scary since they are not tapered at the beginning. I use pediatric dilators that I bought from eBay. I eventually hope to move on to dilatan dilators that are available at CMT medical too.

Yeah, I could probably do better. Those are just the ones that my PT had. The worst thing about them is that they are not perfectly smooth where the round tip meets the straight part.
8/09 - 12/10: fissure, cured by LIS/fissurectomy
1/17 - present: fissure/hemms, healed but pain remained, 3 CRSs (nifedipine, infrared coag) were no help, cold dilating >= 1" helped but still have pain when sitting

William. Thank you for giving this link in a different thread. We seem to be in the same predicament. I only just started PT. I had a fissure between Sept and Jan. It healed, but I still get awful, awful anal pressure and insane pain for about 6 hours after a BM. I have lost all my mojo and all my happiness in life.

Thank you again for the link.

I just started seeing a pelvic floor PT. Only been once so far but she discussed that we will be using a wand for massage, pressure points, etc. She didn’t touch my butt as my fissures are very bad right now but she could feel my muscle spasms though my vagina. She thinks she can help me. I pray she can b/c I’ve been dealing with dehabilitating spams and fissures that won’t heal for 15 months and it’s been the worst thing imaginable. I’m sure you all know what I mean.

CTT - I too just went to PT. We are in the same boat! Mine did not mention booty massage, yet. But I am sure she will. This is getting so unbearable that I am crying every day. I hope this helps both of us.

Many rectal muscles can be treated vaginally, so try not to worry too much about having to do rectal massage.

Will self dilatation ever cure chronic anal fissures..???

I am having large congested internal haemorroids so will it be good for me to do self anal dilatation via dilatator because Haemorroids are active source of bleeding and can cause bleeding while dilatation.

Self dilation could cure a chronic fissure if that's what was needed, which is to get the muscles functioning correctly. It can also help with hems, but if they're currently bleeding then that's probably not a great idea. Perhaps get the hemorrhoids dealt with first before trying dilation.

Here's a follow-up conversation regarding the anal self-massage study. Note that the original authors are clear that the technique was only for acute, not chronic fissures:
