4 month fissure but starting to heal...I think

Hi there!

I have officially finished reading ALL of the success stories on this forum and it is helped me SO much! Thanks so much for whoever decided to organise that thread! So I have had my fissure for 4 months now! I was in France for work, ate way too much cheese and voila! My fissure journey began. I am 31, female, from Australia but living in Spain.

My progress has gone something like this:

1st month- intense pain with BMs but afterwards could go on living as normal. No pain during the day.
- took a few weeks to see a specialist, pain got worse, spasms for hours
- tried lidocaine, no real help, tried fixing diet, small improvement but lots of pain still

2nd month- spasms continue, feeling horrendous
- finally got rectogesic
- helped a lot with pain but still not enough

3rd month- some ok days, some painful
- starting making a food diary, reducing fibre, taking more care
- started self-massage, dilation

4th month- spasms less and less, sometimes 2 to 3 hours instead of 7
- last two weeks of this month no spasms, just tightness sometimes, much less pain or zero during BM
So I am now starting my 5th month! These days I only really suffer pain/discomfort for a couple of minutes after a BM. But it hasn't healed. I don't know if I should try to wait it out longer or go for surgery: Fissurectomy with botox. I don't know if perhaps the scar tissue is too chronic for it to heal 100% and I need a new wound cut through surgery. Any advice is very welcome!

Hi tightbutt,

Glad to hear you've been improving! As my uncle says, "a little better is a lot better."

I mean 4 months is a while, but seems to me no sense getting surgery if it's not causing you anything more than a couple minutes of irritation.

I'm just curious: why fissurectomy + Botox and not LIS? The advice from my doc has pretty much been try to avoid surgery, but if your going to do surgery LIS is the first best thing to try. Just wondering if there's differences of opinion on that or if LIS was contraindicated for some other reason.

I've had mine for about 3.5 months and have scheduled surgery, but I'm emphatically not in the couple minutes camp. I'm prone to back pain and i've actually developed a case of sciatica from spending so much time in the fetal position during this time.

Hey Bloodybum!

I am sorry to hear you developed another health issue from the damn fissure! It's the worst, isn't it?

I only have a couple of minutes pain or discomfort because I am being crazy strict with diet, showering and sitz bath etc after BMs. But if I were to not do those things, I'd maybe also be in the fetal position all day. I want to be able to live normally again so am considering surgery for that reason. I don't know if I am going to heal 100% naturally or if I am just really good and masking the pain and reducing symptoms. My daily routine is a bit nuts and I can't do it for much longer, I am going insane, haha. But I will wait it out a little longer and see...

I am interested in fissurectomy with botox because it doesn't carry the risk of incontinence. While I know the risk is suuuuper low with LIS, if I want to have kids at some point, it might complicate things for me. Fissurectomy doesn't cut the muscle , it cuts out the old fissure and makes a new wound that should heal quicker. The botox does the same as LIS, relaxing the muscle but it's a temporary relaxant. This type of surgery is less successful but I feel it suits me better. I'd still opt for LIS if needed though, as I have heard mainly really positive reviews and success rates are very high.
