Anyone in Canada

Is anyone in Canada able to get referral for colorectal specialist from their family doctor without seeing gastroenterologist?

My family doctor gave me referral to gastroenterologist who wants to do sigmoidoscopy to confirm the fissure.
I don’t want to do sigmoidoscopy if it’s not necessary. I understand colorectal specialist are best for fissure problem.

Please suggest

I've never even heard of a sigmoidoscopy. Gastroenterologists can't really help with chronic fissures other than issuing creams and ointments that may or may not work. The CS is the only one that can perform surgery and botox injections to get rid of the fissure. The constant retears should be reason enough to see a CS. I'm really sorry you're struggling with this.
March 2022 Suffered anal fissure.

April-May 2022 Went undiagnosed because GP couldn't see the fissure.

June 1st 2022 Officially diagnosed with fissure by GI and given nefedapine.

July 2022 Botox injection under anesthesia

I just looked up sigmoidoscopy. Sounds like a waste of time to me. I'm not a fan of sticking any instruments up your butt when you have a fissure. Your gastroenterologist doesn't sound very knowledgeable. This is to be 3xpected because gastros don't specialize in dealing with fissures.. I went to 2 gastro and they were both able to confirm the fissure just by looking inside my butt with no tools or scopes necessary. I hope you can get a 2nd opinion from someone else. The CS specializes in fissures, hemerhoids, colon cancer and other ailments of the anal region.

My CRS saw my fissure IMMEDIATELY without using any instruments like Dust 2 Dust said. A gastroenterologist doesn't even handle rectal issues like this. Sounds like a cash grab. Go see a colorectal surgeon, this is their area of expertise. It doesn't mean you'll have surgery. That is the last option.

LIS, skin tags removed - Oct 2020
Fissure 100% healed - Nov 2020
Still healed as of March 2024

Thank you very much for your feedback.
I spoke to my family doctor and asked for colorectal surgeon referral. She said CRS won’t be interested in speaking to me unless surgery is required. She said If I am not comfortable with sigmoidoscopy, I can try using nifedipine for longer time and see if fissure heals.
She said anyone can see fissure outside and it’s not rocket science (seriously ?? ) and GI is recommending Sigmoidoscopy to see if we are missing anything. She saw little tear back in November but not 100% sure why it’s reoccurring. So she recommended GI for further Evaluation.

So I am planning to use nifedipine for long time like 6 weeks if it reoccurs.
I am hoping it heals. I m taking mag citrate every night too. Finger crossed.
