And you should note, that while there are a couple of Ontario doctors listed near the top, you have to scroll quite far down to see most of them.

Usually, in Toronto, each hospital will have one or two specialists in each medical field (sometimes that is a junior and a senior, but sometimes it's two of the same). You do have to be referred by a family physician, but if Sana has this list, she could at least call to see who deals with fissures (rather than the CRS's that her GP says only look after cancer patients), who might be taking new patients, and then give that much smaller list to her GP. Often, a GP is not willing to do that extra legwork, and will just refer to a general surgeon instead, because it's easier.

In Toronto, most CRS's go under the heading of General Surgeon, but of all the General Surgeons, the ones on the list above specialize in colorectal issues. There are other General Surgeon's who specialize in other areas too, such as breast cancer. I have personally never seen or heard of a CRS who only deals with cancer, most deal with all of the colorectal problems you list above, but maybe Sana's family doctor has.

Interesting that two cities, in the same province seem to be very different on this front.

Yeah that is strange that two cities, in the same Province, that are actually pretty close together work completely differently. I think the only "General" that we have here in Ottawa is your Family Doctor, General Practioner, but they can't even comment on any thing outside of their Jurisdiction which is like Colds and Viruses or something simple.

You just go and tell your GP what's going on and they refer you to a Specialist to have it dealt with.

Then the Doctors that they actually refer you to, the actual Surgeons, are almost always world renowned.
