J-Poucher (IBD) a bit desperate

Hello to all,
I am Alex from Italy
I hope you can help with my fissure.
I need guidance as I'm trying everything but it seems unsolvable at the moment.
I'm planning to see a proctologyst in few days and a gastroenterologist in march.
The pouch is goin quite well but as fecal matter arrive in the anal area, I have so painful cramps I have to go to expel, but I'm not expelling so much and feel not empty.
I worsened the situation straining.

I'm taking Psyllium (maybe to much) it's increasing the bowel movements but I feel better as I don't feel that irritation in the fissure all the time.
Today I'll try to decrease and dring more water as stools seem to be too hard and slow to expel.


Im also trying to take nifedimine+lidocaine but am concerned they are syaing few minutes /hours to work.

I tried zinc oxide and felt things worsened and irritation worsened.

The major concern is the cramps, spasm, urgency to defecate and not feeling empty.
I feel like a prisoner in the last two months as I am not safe going out from office and home.

Hi Alex, so sorry you are going through this. Fissures are so distressing.Many here in this forum use Miralax or Lactulose. Psyllium husks give me stomach cramps. I have IBS. I just got a result from some stool Samples and I have a Shigella/E colli intestinal infection. I start antibiotics tomorrow, really hope that doesn't cause uncontrollable diarrhea. Let's see what happens. Well perhaps you can try Miralax so at least you don't have stomach cramps. Bad enough to have fissure pain. Wishing you the best, and prompt relief
