Hi There,
I'm sorry you are suffering with a fissure, but yo've landed in the right place for information and support. Since you've had a fissure for a long time it is considered chronic. The medication you've been given will give you a better chance of healing your fissure without surgery. It contains a medicine called diltiazem, which will increase circulation and also relax the anal muscle to promote healing. You can increase the likelihood of success even more by making sure your stools are soft and easy to pass with no straining. This is usually accomplished through a combination of diet and stool softeners.
Welcome to the forum. I highly encourage you to read through some of the posts in the General Discussion area to get some great tips on diet, etc.
