BUT i have had no pain now for 2 weeks on a BM have very little spasm at the moment, i am using Diltiazem cream which was given by specialist, before that i was using GTN cream for 6 months, also i am using fybogel twice daily..
as i seem to be improving i dont know what to do, i have my appointment for botox on 27th november..

It may well be that the Diltiazem has alleviated most of the symptoms or even healed the fissure, it depends how long you've had it for? In any case I would go to the appointment and let them do the Botox especially if your still having minor spasms, it can't do any harm and will only relax the sphincter further, the choice is yours, good luck.

Maybe you could ask about LIS instead of Botox, but if your not in a lot of pain and think things are improving then get your specialist to check you over first, he/she might be able to say whether or not the fissure has healed.

I would just see how things go. If you feel things are healing up down there, best to let sleeping dogs lie, and not mess with it. You can always cancell last minute. I can appreciate its a tough decision to make at this point.

I agree with omb. Diltiazem worked for me.7 years ago. No problems until recent constipation. Was prescribed nitro ointment which did nothing. Switched to diltiazem a week ago and great improvement already. Also find coconut oil very soothing. Stick with diltiazemfor a few weeks at least would be my advice. Time for other interventions is if it doesn't work.
Best wishes for a good recovery.
