You need a prescription for either nirtroglycetin or nifedipine (or diltiazem) to help relax the sphincter and release the muscle tension on the wound. Some people find it helpful to also use a local anesthetic like lidocaine or benzocaine, but this has never helped me. It only stings and irritates more. The key is the soft BMs, no straining, a prescription topical, and the warm baths. Alternatively, short applications of ice insulated by a washcloth to avoid tissue damage can really help the pain.
If all conservative measures fail, then Botox or surgery is the next step to start considering.
So sorry you are hurting so much and that your doctors are treating this so casually. The pain of fissures is life altering in every way, and needs to be addressed seriously.
