You said that you would be seeing your doctor tomorrow. The best doctor for anal fissures and anything to do with them is a colon and rectal surgeon (CRS), so if you are not seeing a CRS, you might mention to your doctor that you would like a referral, or make an appointment yourself.
Having had your fissure since October last year, and with it healing and then reforming, you probably have scar tissue, which means a tightening of the anus, hence the bleeding when you have sex, starting the cycle all over again.
A few of us, unable to have the surgery that is often recommended with an anal fissure (Lateral Internal Sphincterotomy, abbreviated to LIS, regarded as the gold standard) do anal dilation, slowly working to stretch the muscle. It is a slow job, but it works, and is useful for those of us with no other choice. You have to have patience...and you need to explain that to a partner, IMO.
