think I can help. My advice is to step out of the cycle of worry and doctors, for at least a week and see what happens.
I, too, had been recently suffering greatly from delayed rectal pain after bowel movements - lasting sometimes the entire day so that even my tailbone and upper thighs ached and felt weak. My life was turned upside down by this for over a six week period. Worry? You bet. Doctors...not yet.
Instead I researched the best source on this there is: online blogs by other sufferers from this same condition - such as yourself. I then began the following regimen that cleared up this condition - completely - within three days.
Went to the supermarket and purchased the following items:1. California Pitted Prunes 2. Organic Psyllium Fiber (Konsyl Brand) 3. Generic Stool Softener 4. Organic Flaxseed Meal (Bob's Red Mill Brand) 5. Bottle of Witch Hazel 6. Hemorrhoid Cream
At the outset, snacked on the prunes, eliminated beef, sugars, cheese, bread (not entirely), etc. from diet, loaded up on vegetables, fruits, probiotic infused yogurt and drank and continue to drink spring water throughout the day - I cut down on caloric intake in general.
Twice a day, large glass of water with the Pysllium Fiber. One Stool softener tablet (100mg) per day.
After first bowel movement and shower soaked my rectum thoroughly (flat cotton swabs) with warm (not hot) water and coated rectum lining and surface insides with the witch hazel (advised by a anal fissure specialist online); and swabbed rectum surface with hemmorhoid ointment before dressing. Repeated this process before bedtime.
Ate a few bowls, daily, of organic raison bran and oat cereal (low calorie) sprinkled with two tablespoons of the Organic Flaxseed Meal. Also ate probiotic yogurt, raw almonds for snacks, salads and fish for meals, water, water, water.
Cut back on caffeine and alcohol (one cup java, afternoon tea and one glass of wine per day).
Stuck to this regimen and procedure and will continue to do so - the prunes are a must throughout the day....and water.
The first morning after beginning this regimen I awoke to my clockwork, post-coffee bowel movement, this time before my coffee, and defecated large and soft, but solid. Next day an unusual second bowel movement, as large, soft and solid as previous day, followed an hour after an initial soft bowel movement. Pain free the entire two days for the first time in six weeks. Third day, one bowel movement only but again, large, soft, solid. Pain free, feeling fully restored, energy revitalized and general feeling of intestinal emptiness not felt for months.
My conclusion is that my (and perhaps your) rectal pain was caused from blocked feces within the colon - built up over time due to poor diet, stress, etc. - and that triggers long lasting spasms when we are in the cycle of incomplete bowel movements - and that we may even believe are complete. This, in my view, exasperates anal fissures, hemmorhoids, etc. and over the long term can develop into a serious problem.
I am 56 years of age, active and fit (running for 30 years) and, of course, dealing each day with common stress, professional and family problems as we all do....But, we can maintain and restore our health with self-awareness and common sense approach.

That's an interesting point about the accumulated blockage over time causing/exacerbating fissures, because the best I've felt since having my fissure was the day after doing the colon prep for a colonoscopy. Maybe this was because I fully cleaned out my colon.
