New Here.. and in PPPPain..

Hi all.. give me your opinion.. I am 33 yr old Female
Monday- 5 days ago- I was on a conference call and had to poo a serious one... but could not because I was leading the call and talking so I had to hold it.. during me holding it.. I felt like a slight stab and then the poo sensation went away.. From there it's all been downhill. I started having burning pain in/on/around my anus all day and night. Felt a hem (I have had hems before and never ever this painful at all). Ibuprofen and prep H made it very small. Taking Ibuprofen would subside pain some but not 100%. So basically the only time I would get relief was when I laid on my stomach and did not move. After Going Poo was a whole other ballgame the pain would roar through.. pressure/burning/stabbing/throbbing for about 2 hours afterwards totally unmanageable.. I will be in tears and want to throw up the pain is so bad and makes me anxiety ridden.. before going back to the slightly bearable but still painful pain.. I have No Itching or Blood though.. So I went to my GP because the pain is so bad thought for sure something more serious and she saw the hem and said she looked for a fissure and there was not one there.. She gave me analpram.. this was 24hrs ago.. I do not feel hem anymore but still painful to sit or do anything and boy oh boy after pooing Horrible Horrible.. only thing that releases pain is passing gas and just waiting for 10 pain to go back to 7 pain.. any suggestions/thoughts out there?

Hi Ouchness,
Please go back and get referal to a CRS, NOT a GP and if you can, as soon as possible.
Firstly, a "standard" GP does not have the expertise of a specialist Colon-Rectal Surgeon and a) may not even know what they are looking for with respect to a tear and b) may not even be able to detect one through a standard visual/ internal examination.
If what you suspect (and I suspect) is a fissure, it is vital you seek the necessary medical support quickly.
Just like yourself, my tear never bled and was not able to be visually seen by my GP, which is why I immediately opted for private referal to a CRS.
I undertook an emergency Examination Under Anaesthetic (because of the sheer pain levels at that time) and sure enough, the fissure was found!
I was then treated immediately with a course of Diltiziam cream, which is prescribed specially to reduce spasms of the internal sphincter muscle and allow the fissure to heal naturally.
The main thing for you is that you seek the advice, diagnosis and treatment you need by a CRS as soon as possible to start your healing process.
In the meantime, start eating all the fissure-friendly foods you can, drop the deep fried/ spicey/ processed, "white" foods/ convenience rubbish and drink LOTS of water to ensure you produce the smoothest, softest BMs you can.......
Start on a Stool Softener too.
Pre-lube every BM with Vaseline to allow everything to pass out the anus more easily and continue to use Prep H/ Coconut Oil (whatever works as relief afterwards) Try not to strain in anyway when you go to the toilet!......Ensure you take "Sitz" baths or simply sit in a hot bath tub after a BM for a while for relief and to encourage blood flow to that area. Take painkillers if you need to too.
Scour this site for all sorts of fantastic tips on fissure-friendly eating/ Stool Softeners/ Methods to assist in pain relief - there are an abundance of options on here, which help a LOT!........
All the very best to you in dealing with this - Stay strong & positive, be strict with the above and fingers crossed for you that you will be able to work through this quickly and resolve your issue as soon as you can :)

This is SO like what happened to me, ouchness1.
Please, go to see a colon and rectal surgeon, and as soon as you can. Do not leave it for anything. It is, if you have a fissure, only in the acute stage now, and can still be stopped in its track before it progresses. In about eight weeks, it becomes chronic and much more difficult to heal.
If I had known what would happen to me when my saga began over two years ago, I would have run to a colon and rectal surgeon (CRS). Image
