:smilyhug: It sounds like all good sign's to the further road to healing..I also went through that all, and you also will heal. it sounds like your still travelling that road.

I don't even need my Nitro any more, I have two more repeat's just in case. Be patient..time. But I can see your getting there. Keep up with the healthy eating, and what ever it takes, fibre and ointment.

Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil

Just checking in to say - everything is still fine! My family went to the Appalachians last week, we hiked every day - not far, about 6 miles a day, but enough to challenge my butt, and I had no pain other than a little surface burning.

I wore leggings under my skirt and was warm and comfortable! I sat on a hot water bottle every evening while we watched movies. It was very cosy.

Now we are home, I'm wearing jeans again, and feel fine. Again, a bit of surface burning every now and then, but nothing to worry about. My sentinel pile seems to be shrinking. The painful hemmie seems to be gone. I am so happy. I've got a full weekend of dancing coming up, and I'm not afraid at all.

And (sorry if this is TMI) I've resumed my sex life, which was down to almost nil because everything down there just felt so yucky, that even when I wasn't in pain, I just had no desires at all. So it's a huge relief to have my normal feelings back.

As JHH said, I have my life back. It's not exactly the same life, because 1) I now drink far more water, and I'm really careful to get enough fibre. and 2) I know how lucky I am not to be in constant pain. I will NEVER forget how lucky I am.

A new twist: my bum is itching quite badly again!

I was thinking of stopping the nitro, since I haven't had any pain other than slight surface burning for weeks now.
For several days I've been itching slightly around the anus. Not inside, but the sensitive skin around it.
I didn't use any nitro last night, but did go to bed with a hot water bottle under my bottom, like every night for weeks.
In the morning, I somehow couldn't resist. I had to rub the itch, and I did. Of course that made the itch worse.
What was worse was that the skin seemed to seep out a dampness, just like before.
Again, I didn't put on any nitro. Instead I used a lotion my GP recommended when I had my diltiazem rash. It's called Sarna. I must say, it's very soothing, and the itch hasn't bothered me today.

I'll try giving up the hot water bottle. I don't need it for pain, but I find it comforting. Still, it may be contributing to the itch.

Ha ha, the ramifications of this fissure seem endless!

lol...and on it goes..even to this day i love my heating pad, it turns off after two hours, just in case I fall asleep.

It sounds like your right on the healing track, I still use the sudocrem.

How long?
It's hard to say because it's such a roller coaster. You think you've got it licked, and then you get a set-back, but it's not as bad as before. So when do you consider yourself on the mend? When do you consider yourself cured?
For instance, today I carried something heavy to work - about a mile's walk. So silly of me, but I hate to drive when I can walk. Ever since, I've had some twinges in my butt that I never would've had before all this started.
I feel 99% better, but that last 1% is elusive.

Still, to sort-of answer your question, Stillhoping, I got my fissure in mid-May. I was in a lot of pain until the end of July. Healing started, with lots of set-backs, when I started the Diltiazem towards the end of July.
I don't think I had any more spasms after mid-August. There was still soreness, surface burning, and twinges, but the agonizing spasms were over.
It's been gradual improvement ever since. I just have to remember not to over-do things. The twinges are an excellent reminder.

Yes, roller coaster. That seems to be a an appropriate analogy for the healing process. It seems that each time you think you are making progress, something happens and you realize that you are not there yet--or not as close as you might have thought. I am on the path to healing, but I have thought that I had been close before. It is very easy to get discouraged--plus its not the sort of affliction that you tend to talk about when some asks "So how have you been?" At this point, I almost don't care how long it will take(hopefully months not years!), if I know that it is possible to heal completely and eventually be back to normal. How long did you use the diltiazem, what strength was it, how much did you use, and how often did you apply?

Stillhoping, I used the diltiazem for around 3 weeks before I developed a really bad rash (see my posts above). However, before the rash, it did seem to help healing. Probably the diltiazem was responsible for the end of my spasms.
Because of the rash, I switched to trinitroglycerine in mid August, which also helped, and didn't irritate my skin. I was still having some pain and soreness into mid October, but very little at this point.
