Colonoscopy for Crohns?

Hello,

As you know I am planning to get LIS soon, but suddenly today my local CRS suggested to have colonoscopy prior to this, suspecting Crohns! Where he was 9 months??!!!

I wonder if somebody passed this procedure, as I am afraid it will make my wound worse. How wide the tube is?
Also there is huge preparation for this procedure, I have to take Mineral Oil and do not eat for 24hours.

I just wonder if there is also some another way to diagnose Crohns? Or colonoscopy is most reliable ?

That messes my plans, and I booked this procedure for this Monday...

But probably if I have Cronhs, I don't need LIS? :dunno:

Mineral oil as a colonoscopy prep? Never heard of that before. But if you are going to have a colonoscopy it seems like this is as good a time as any because you already have surgery scheduled. Not sure how big the colonoscopes are in your country, but in the US they aren't much bigger in diameter than a finger. No guarantees on no more damage to the wound with the procedure. It does happen. The prep for the procedure involving cleaning out the colon can also cause problems for the wound. There aren't really any other good tests for Crohn's unless a doctor suspects the disease is present in the small intestine, so if you aren't having any symptoms to indicate small intestine involvement, you would need the colonoscopy.

Do you have any other symptoms of Crohn's? Diarrhea, abdominal pain, weight loss? Crohn's can occur just in the perianal area.

If you do have Crohn's it does kind of complicate the decision for surgery. And not all doctors agree on whether LIS should be performed on Crohn's patients or not. The problem is it can make healing of wounds more difficult for some people so it is a good idea to rule it out.

I just want to say that the colonoscopy I had (soon after my fissure) gave me no problems at all. The cleansing was fine because my stools were so loose and watery they didn't hurt, and the procedure itself was painless. It didn't tear me more.
I hope your experience will be as trouble-free, Igrik. You deserve a break.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Thank you both sharing your experiences.

I don't have any other symptoms of Crohn's, no diarrhea ever, no abdominal pain, and since I got my unhealed wound management on track I started to gain weight(already + 4 kilos).

I have only one symptom it is unhealed wound after hemmie excision. And to be honest I am afraid to have colonoscopy under GA, as I will have LIS at 11th of December. Don't think two anesthesia's are good in such short period.

Also I don't want fasting for 2 days, and mineral oil clean ups. Also results of colonoscopy will be on my table only on 9th, but on 10th I have plain already :) So it will be last minute cancel if Crohns persists, if they do, though I might not need LIS. Brrrrrr...

It is quite messy, also I was informed about risks of colonoscopy, such as colon injury and possible bleeding.

I Crohn rare disease? Are fissures more common than Crohn or vise versa?

It's a tough decision to have to make so close to surgery. Certainly fissures are more common than Crohn's. That being said, however, when the fissure is failing to heal on its own one of the things that is suspected is possible Crohn's disease. Without any other symptoms or suggestions of Crohn's in your case, the statistics are in your favor that it isn't Crohn's. Trouble with statistics though is that there is always someone who falls in the rare percentile. As far as anesthesia is concerned I wouldn't be too concerned about the two procedures being close together. The level of anesthesia used in both cases isn't very deep at all, and since your health is generally good, there isn't much to be worried about. Tough decision to have to make. One of those times when you just have to go with your gut instinct.

My general physician made me have blood test done to look at IgG and IgA level. It came high and he suspected me to be having Chron's. My gastreo agreed to do colonoscopy and endoscopy to see whats going on. Though my gastreo told me blood level doesn't assure or mean that i may have Chron's.

My case is different, i had my LIS 6 months before and my fissure is almost healed. But i have severe pelvic pain (non bacterial prostatitis) triggered by the fissure stress. I was given sedation and i didn't feel or know a thing what happened during colonoscopy and endoscopy. To my knowledge and experience, you may feel the pain while having the colon cleansing as you have the fissure in place.

BTW let me know which cleansing preparation you have been suggested? I had to go to the restroom 25 times. I know how it is to have a bowel movement with fissure. I went through both Moveiprep (costly, bad taste and gave nausea) for my colonoscopy/endoscopy and Miralax (Cheap and as effective as Moviprep) for my pillcam. My GI told i don't have any issues other than acid reflux.

If your GI specialist is ok with doing the colonoscopy then you can go for it as anyhow you will have the LIS soon. Or you can have LIS and then after few months go for colonoscopy. Decision to make based on your comfort and fissure pain you are dealing with.

May i know why you want to cancel LIS - if Chron's is positive?

I decided to have blood tests first, to see if there are any indications about Crohns.

My logic on decision is following, if I have Crohn, possible I have to treat this instead of cutting muscle, which is permanent procedure. And maybe I don't need LIS, do you have another concern here?

As i said - blood test doesn't assure or mean that you have Chron's. My blood test level was very high and i was shocked. But i didn't have Chron's. My GI told me - they usually do blood test only if Chron's comes as positive through colonoscopy. That's why i even went through pillcam to rule out any other issues. My GI was disappointed with the approach followed by my general physician.

I don't have much insight on Chron's with fissure. Did your GI or CRS assured that treating Chron's will cure fissure and take the pain off? And also about the fissure re-occurrence in future?

Hi I had a colonoscopy with an active fissure too. There is a smaller camera that can be used for this. I had mine done before LIS to rule out anything else. Turned out I had a polyp which tg was nothing to worry about it was removed there & then. The prep I used was klenprep it was gross but I got it down. Going to the loo wasn't too bad just use Vaseline to protect area. The anaesthetic used for colonoscopy is usually just light sedation I woke just as they finished up.
You should go ahead with it, it's great to know nothing sinister is going on in there.

Here are two recently introduced stool tests which can help diagnose an IBD (Chrons or Ulcerative Colitis).

They are much more accurate than a blood test but not as accurate as a colonoscopy which is the gold standard. You should check with your doctor if they are available where you are.
