I have posted multiple times on here about the dramatic effectiveness of an antibiotic: metronidazole 500 suppositories, one a day at bedtime for 10 days. It did not cure my chronic fissure (it kept coming back), but it provided astonishing pain relief almost overnight and helped wound healing as well - i stopped seeing blood after 2 days of use. The fissure was not infected; the use of this antibiotic was more for its anti-inflammatory properties - and, who knows, maybe the antibiotic aspect helped as your link suggests.

ap093 and appleapple, I know that this is a very old discussion, but did either of you ever try the Betadine treatment? I have recently learned that it might help me with a very painful bout of ulcerative proctitis, which just happens to be in the very same area as most fissures. I will be seeing my surgeon next month and bringing this topic up with him. I will post here again if I learn more.

Know nothing about Betadine but it sounds suspicious. Try metronidazole suppositories! You have nothing to lose and they might change your life. Again I’m surprised no one has ever posted about this novel treatment (cure for pain, and rapid healing, though not necessarily full cure for chronic fissure).
If it’s gone on this long maybe time for fissurectomy with advancement flap surgery, sphincter sparing. Worked for me, I’m 5 years out. If it ever comes back I will immediately zap with Flagyl suppos.

Thanks for adding your experience. I have had my proctitis for about 12 years now, and have been on Metronidazole twice in that time. It had no affect on my proctitis, but may be a way forward for fissures.

It has to be suppositories because the anti-inflammatory effect is *local*. Pills won't have any effect. Hope it helps someone bold enough to try.
