I had this combo, and yes it sucks. Im sure my hem came first and caused the fissure. The doctor wouldnt do anything about it, since it wasnt thrombossed enough to be cut.
Anyways, I kept my stools soft, and the hem disappeared after a few weeks. Unfortunally the fissure did not, and Im still suffering.
If its badly thrombossed and relative new, you can have the clod cut out. Otherwise it will dissolve by itself in time. Keep your stool soft, and learn to squat. Thats about it I guess.
BTW. For me the throbbing from the hem was noting compared to the burning and spasms from the fissure.
