Pain several hours AFTER defecation - is it AF?

Hello, I see occasionally, after a thick bowel movement, blood in the toilet. It does not hurt to poop, but then after 2-4hours, it starts to burn and hurt like hell, and it continues for many hours or days.
Is it anal fissure?
I have been having hemorrhoids for years and when they got worse, I had a ligation. It was for some reason extremely painful for weeks after - my guess that the doctor did it below that magic line.
Anyway after that I started having this problem with occasional pain after BM.
I do not want to go back to proctologist, because he uses a sharp metal tool and twists it, then it heals for weeks.

I think any procedures in this area can result in pain after BMs, whether it’s a fissure or just some sort of sensitivity post op. It sounds like it could be a fissure. I’d go see another doctor who does not hurt you on examination. I’m on doctor number 3;) I had similar to you - small surgery for skin tags and then terrible pain after BMs which I’d never had before. I hope you are feeling better? Just make sure you keep everything soft (but not too soft!!) tricky…

2nd opinion Jan 2023 fissure diagnosed, diltiazem - allergic
recommended Botox Fissurectomy
3rd opinion - dilation 2x per day plus GTN ….
