Thank you all one again for your replies -- the are very, very helpful. The itching isn't more outside -- it is a very deep, annoying itch that prevents me from sleeping entirely. But you're right: it's mostly itching, not pain, and that makes the fissures diagnosis puzzling.
The doctor was a hemmorhoids expert, and he evaluated me very extensively (looked inside with some kind of special camera). It was in his interest for my problem to be hemmorhoids, as that would allow him to do a procedure (using a kind of UV blast to burn them off), so the fact that he said it wasn't hemmorhoids I think is pretty reliable.
It was in this context that he said it was likely fissures. I guess I'm still curious if it is pinworms -- do you guys know if they can go away for a few months, then come back for an episode of 7-10 days, then go away again, all without any direct treatment of them?
