Hi, I can sympathize because I'm dealing with the same thing. There are only two things that I know of that can help. Dilation which they don't use much anymore in the US or advancement flap surgery. The flap surgery takes healthy tissue and folds it over into the anus to cover where the fissure was and to make the opening larger and more elastic to help prevent future fissures. If you read about it through medical journals they say the outcomes are good, but if you read about it in forums like this you'll find some pretty scary posts. There are a few people here who have had really good outcomes but lots of times they don't post once they heal. I'm going to go to the CRS in a few weeks and finally discuss the advancement flap procedure. My biggest problem is pencil thin bms ( that's as big as I can pass) and that awful Charlie horse up the butt feeling I sometimes get, to me that is the worst pain. I live in fear of using the bathroom so I'm to the point where this has overtaken my life and I want to try and get it back. I've been battling this since the beginning of 2008. I thank god for miralax and a heating pad but I don't want to spend the rest of my life like this if I don't have too. Whatever you decide to do I wish you the best. Suzy
