What seems to be irritating you the most is the itch, so you might ask your doctor to test for candida. An anal fissure's pain is something you cannot mistake; think birth. This website has some good info.

Personally, I would stop using anything with witch hazel. Often they have alcohol in pads which I think just makes things more sore. Just use water. Never ever rub. Use Cottonelle loo paper, with aloe and Vitamin E. I use plain water and no soap "down there". I get plain sterile gauze pads, wet them, to clean. Again no rubbing.
You could have a less symptomatic fissure, but you
need to see a CRS (A colon and rectal surgeon is, by far, the best doctor and believe me they have seen it all...) to be able to get a proper diagnosis.
I expect you know about everythig else - no spicy food, don't get constipated, enough fibre for your height and weight, plenty of water.
