For those who are reading this in the future, there are three main costs associated with lis:

1) $750 - $850 for the actual lis procedure
2) ~$2000 for one day at hospital
3) ~$400 for anethesist
= say $3250

Btw, botox will be about the same except the cost for the actual botox injection is about $550.

Waiting times to see private surgeon is around 1 to 2 months. I don't know how long the wait is to see a public surgeon but I've been told its 6 - 12 months!

Botox seems to get the best results when done within one year of the fissure first occurring. In other words, if you haven't healed after say 8 weeks and u want to get botox, get a referral to see a commercial surgeon as a public patient asap so that by the time u actually get to see the surgeon it hasn't been more than a year and u have a better shot of healing the fissure with botox.
