You really need to get an emergency referral and then demand a LIS (instead of more ointments). Where are you located?
Fissure since about 2007
Fissure diagnosed in 2011
Diltiazem for two years - didn't work well
LIS January, 2015
Hemorrhoidectomy December, 2017

Have you considered dilation? Stretching the muscles can help with the spasms.
8/09 - 12/10: fissure, cured by LIS/fissurectomy
1/17 - present: fissure/hemms, healed but pain remained, 3 CRSs (nifedipine, infrared coag) were no help, cold dilating >= 1" helped but still have pain when sitting

I’m in greater Vancouver. Maybe I should just throw down the credit card and see a CRS at a private clinic.

I’m frustrated because this condition is never viewed as serious by doctors.

Dilation sounds reasonable, who does that?

You can buy pediatric dilators online and many on this forum use them (use the search button above to find out more). Before going private, I would ask your GP for an emergency referral. You have to tell him or her how debilitating your situation is, and not leave their office until they understand!

You can find pediatric dilators at this link: https://www.expressmed.com/product/diag ... r-set.html. They're not cheap, but I found them very helpful though I'm not painfree yet, and I've moved up to the larger Dilatan ones. I started at 8 mm, and it was all I could handle at the time. I also found Aloe-cadabra lubricant to work well with dilators because it spreads well unlike regular lubricant.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
Currently in pelvic physical therapy

Yea that's what I told my mom my fissure is like, see that fire coming out of the ground in Hawaii? Yea that. I haven't gone through your post history but if the pain is that extreme I would definitely go to a colorectal surgeon as others have mentioned.

To aid with the spasms I would eat less (less poop = less pain ), you can get an emergency 'nifedepine + lidocaine' ointment at a compounding pharmacy, helps if you know the pharmacist. This brings blood flow and relaxes the sphincter, also a flexible shower head attachment to rinse your butt with hot water to supplement sitz baths. Epsom salts in the sitz bath helps as well.

Could you not insist on having a LIS? It makes such a positive difference even when it isn’t completely successful. I don’t regret mine for a second.

Painkillers don't help, even Tramadol. My biggest issue is spasm. The sphincter is stuck in spasm all the time.


use magnesium citrate pills , those are for relaxing the muscles and are known for making your bowel into soft easy crap .
