Mysterious rectal problems - not sure what to think?

I've come here in the hopes that maybe some of what I say might ring a bell with someone here, because more than anything I'd just like to know what's going on with me.

Problems started after taking a strong antibiotic called Ciprofloxican. It began with a minor itch, which soon became a major itch, and even more worrying, a feeling like something small (like a raisin) is stick in my butt. I've never really had pain, or any bleeding, but the itching was terrible for some months before I went to the doc. Worse every time I had a BM.

Had first exam, found no evidence of hemorrhoids. Went to another doctor, she said it seemed like a fungal infection, since I had itching, and redness of the skin. As for the feeling of something "stuck" back there, her only explanation was intertrigo. Although I don't think intertrigo would feel like something is stuck in my butt at all times, sitting, standing, walking, and lying down.

Anyhow, I took fluconozole and ketoconazole I'm still taking daily for this supposed fungal infection. It's been over a week, and while the itching has significantly decreased, the feeling like something is stuck in my butt persists as strongly as ever. It's driving me crazy. I cannot pinpoint exactly where the feeling is coming from. Sometimes I think it's in my anus, or the perianal area, or maybe even on my buttocks. Spreading them apart seems to make the sensation quit. I've often wondered if it could be a fissure, or some kind of scarring? I just feel like if it was completely fungal it would have been knocked out by now.

Another strange thing I've noticed through this whole ordeal, is that my BMs (other than lacking normal solidity) is that they have often had tiny white specs showing up in them. Which is something I never noticed before. I don't know if that's fungal, or a sign of something else. Just about every possibility had gone through my mind many times over the past few months, tumor, prostatitis, fissure, internal hemorrhoid, parasites, etc, and I feel like I'm not really getting anywhere, and worrying if I can afford to being that I'm not insured and have to pay for everything out of pocket.


Does anything sound familiar here? Does anyone think my problem really is just fungal? Is it possible fungus could cause a sensation like that? How long should I wait to see if it goes away? Or should I run straight back to the doctor and demand further explanation?

Itching can sometimes be a symptom of a fissure which is what you may have instead of a fungal infection. Have you been to a colorectal specialist to get it checked out? I had the feeling of something stuck​ in the butt after a 30 hour international journey and I assumed it was an internal hem or something. It went away after being active for a while.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
