Itchiness is the number one symptom of "External" fissure. However, it could be aggravated Hemorrhoids, as well. While a "fissure itch" is temporary and should be felt only on the day when the bowel movement is large and bulky; "Hemorrhoid itch" is permanent and will be felt everyday no matter your bowel conditions. Don't know about rash though. It could be anything -- clothing, hygiene, cream allergy or inflammation.
To overcome itchiness, a sitz bath (hip level) with Epsom Salt for 10 minutes twice (after a BM and before going to bed) for three days should help. Wash off the salt and apply Vitamin E gel (break open the capsule) in that area to speed up the healing. Please note, Epsom salt contains an anti-inflammatory ingredient - Magnesium Sulphate, which helps in disinfecting, cleaning, muscle relaxing, and overcoming inflammation.

You may either take an enzyme supplement or find natural ways to activate the enzymes in food. Soaking seeds/grains/nuts can activate enzymes in food. (For example, if you need to consume brown rice or kidney beans, you may soak it for 6 - 12 hours to activate enzymes before cooking and help with digestion)
You also need pro-biotics. Please consume plenty of plain yogurt (avoid if you are lactose intolerant) - a common and NATURAL source of obtaining Pro-bio tics in your diet.
