Hi,
This forum has been very helpful and I'm trying to convince myself on having the LIS procedure done.
Here's how it all started started, my family has a history of Hemorrhoids and I've had them too since I was 10 yrs old (I'm 32 yrs now/Male), the last 4-5 yrs I've been having pain on and off and I was under the assumption it was my hemm's. Visited many Gastroenterologist's in the past but everyone suggested a proper diet routine and said it would go off, a couple of years ago I had a colonoscopy done and they identified an ANAL FISSURE, I was given the nifedephine ointment and it helped. But again after a few months it would come back. I still thought it was because of my hemorrhoids. 2 months ago the pain was unbearable and I went back to my GI and he prescribed Rectiv(Nitro), had headaches and the pain still dint subside. Last week met a CRS, he put me on nifedephine and other tropical lico which seem to help a little but from last week the pain is 10+ during and after a bowel movement. I feel like my entire intestine is popping out thru my anus and cannot tolerate the pain anymore. My CSR asked me to be on these creams for a few weeks and then decide on having a LIS but I think my tolerance for pain is so low that I cant deal with this anymore. I also did notice that my fingers cant reach inside while applying the ointment, looks like the sphincter muscles are really tight from the spasms.
I've had a few surgeries in the past and I'm a little worried about not being able to be 100% post surgery. I had a shoulder surgery and I don't feel any better with it now. The CSR suggested that it wouldnt take me more than a week to recover and be back to work. I'm in IT and work as a consultant and can't afford to take too may days off as I wont get paid, he suggested that the surgery be done on Thursday and I could get back to work as soon as the following Tuesday. Is that possible ??
Also does getting the surgery guarantee pain free BM's and getting back to a normal life ?? I so can't wait to be back to normal. I don't remember the last time I had a peaceful sleep in months.
I've made an appointment with the CSR for Wednesday (03/13) hoping he would be able to fix a time for the LIS this week. I need all your prayers and support.

Sorry to hear your troubles :(
IMHO you are an ideal candidate for LIS. It heals 95%. Getting back to work within a week, is probably not going to happen.

P.S. You should do some reading in this forum. Plenty of stories from people that had the procedure done. Also be aware LIS can cause continence problem in a few %.

Sorry to be the downer here, but you really haven't given the creams a chance to work. It takes a couple of weeks before they really kick in. At first, no one can put them inside. Just rub it around the outside until you loosen up a little.
I just had the surgery last week, and let me assure you that it is no quick fix. Its not something where you should be thinking that you don't want to wait for the creams to work so you might as well speed things up by doing the surgery. Ha! Really. I'm on day 5 and I'm still hurting more than before the surgery. Its getting a little better every day. My brother works in a hospital for colon cancer specialists. They told him that this is one of the most painful surgeries that they do. It is very successful. My doctor says that I'm healing just fine and there is every reason to believe that I will heal completely. But I would not have done this if I could have used the creams. (I'm allergic to all of them.)
I'm back to work today, but I have a very fissure friendly job. Not too much sitting and no heavy physical demands. I work from home, so I even fit in a nap.
Also, even with the surgery, you still have to change your diet long term. There is no quick fix for this stuff. It just sucks. We all are going through it, and people do recover. From time to time, people even have these miracles where everything turns around in a few weeks. But for most of us, its a slow, steady healing process.

Coconut, If 8 weeks on nitro doesnt help him, I dont think more time will help...

I'll give you that. But strictly speaking, he could have only tried it once, two months ago and then given up. I added a disclaimer to my post. :)

I've had the fissure for more than 2 yrs, the pain keeps coming on and off, and I also had profuse bleeding for quiet sometime. Coming back to using the creams, I was using Rectiv for a month but after my CRS prescribed Nefedipine I've been using that and yes it's only been 5 days since I started using them. But what I noticed is after last weeks appointment with the CSR the pain during and after BM have increased a lot. Last 2 days has been unbearable and I dint want to go thru with this pain for the next couple of weeks. I have been using Miralax but not found the right level I need to consume yet, somedays I feel constipated even after taking 1 CUP and I take another cup the same day, this produces too much of BM and inturn hurts more than being constipated. Wish I was able to come up with a proper dose of laxative since my stools are still a little harder. They are never consistent
