"Two New Items"...

...when outside "icepacks are a lifesavor for the burning pain" to sit on, yes I look 'stupid' snears from people, but what works for me is "sitting on ice outside".

Secondly, try to get rid of all the "leftover poo inside' this is very difficult through the daytime, I find even a little inside irritaes the AF to death and burns. Maddening!! Since October 8th and still suffering. No more stool softner for me, do not like pooing 24 times a day from the softner. Just one teaspoon of Metimucial a day, otherwise I am pooing all day lone, which i cant handle. And yes everyday is different in the quest for pain free. Thanks

Does ice packs really works? I have heard cold is very bad for fissures. When I was at the worst of my fissure, or even today when I feel irritated. Sometimes I would like to put something cold in my bum, but I am afraid that I will make things worse.

The ice probably numbs the pain and reduces inflammation, but it would also reduce blood supply to the anus. The anus is already deficient in blood supply, and the high anal resting pressure most fissure sufferers have, reduces the blood supply still further. (squeezes the blood vessels).

You need good blood supply for healing.
Canada, I know your hot water bottles have burst, because you are a big guy, and you sit on them. Maybe you could try lying on your side and resting the bottle against your butt, or lying on your back and having the bottle under your buttocks but with little weight on it. Or fill the bottle only slightly, and sit on only one little part so that most of the water is in the part you are not sitting in. Or you could try an electric heat pad.

For sprains, they recommend alternating heat and ice, and maybe you could try that.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

Actually if the ice gives you relief, just make sure you aren't sitting on it for too long as it can cause tissue damage (no longer than 5 to 10 minutes at a time). As far as circulation concerns, it is true that during the application of ice you will see a shunting away of blood flow from the area. But upon removal of the ice, the area will experience hyperemia (a surge of blood flow to the area), thereby increasing blood flow and giving good circulation..

The only other thing to be cautious with in using ice is to watch out for the possibility of increased muscle tension in response to the cold temperature.
