IBS and Anal Fissures

I am looking for any help. I have IBS and anal fissures. The pain is greatest in my rectum when I have D. Please help me. This is driving me crazy and can't find relief. My doctor has told me to limit fiber as he thinks it's bad for me. Any help is greatly appreciated!

Hi Paininthebutt,
I'm sorry you are in so much pain. What do you mean "D"? Do you mean diarrhoea?
You'll hear different things about fibre. If your doctor knows you and is familiar with your IBS, he could be right - but most doctors would tell you to eat more fibre with a fissure. It's a bit controversial though.

The most important thing for you is to get regular soft but not liquid stools, not acidic, and well-formed. You might have to experiment quite a bit to find out what works. Keep a diary of what you eat and how your stools are.

There are lots of people on this forum who have IBS who will have advice to you. Also, do a search of this forum for IBS and see what you can find out.

You should also see a colorectal surgeon. They generally know a lot more about fissures than regular doctors. You should probably be prescribed nifedipine or diltiazem or nitroglycerine. These ointments relax the anal sphincter, ease the pain and increase blood supply to the anus to promote healing. Again, you may have to try more than one to see what works for you.

In the meantime, a painkiller like ibuprofen might help (if you can take that). Also, sitz baths, full baths and/or sitting on a warm hot water bottle can be soothing and help heal you.

Yes I am sorry I meant Diarrhea. The burning is tremendous with diarrhea but not regular stools. I have been prescribed the nifedipine creams and lidocaine. not much helps. I am afraid to eat at this point

How long have you had IBS?

just diagnosed a few weeks ago. but have always been fairly constipated. seems like I get constipated then it goes to diarrhea. the anal fissures only hurt during diarrhea. Why would that be. also lots of cramping and pain before bowel movement., after I go the pain subsides until the next wave. been all through the tests. endoscopy, MRI's, etc

That sounds horrible. I would definitely do lots of research on diet. I don't have IBS, lucky for me, so I can't really suggest anything. Hopefully you can find a diet that will help you.
I hope other IBS sufferers will look at this topic.

Are you taking any other drugs, like Miralax? I know sometimes Miralax irritated me more. I still take it in relatively small doses, though. I also learned that some raw vegetables really bother me, too, so it's usually best for me to eat whole grains and usually citrus fruit.

Thank you VDB324. yes those foods bother me too. I don't take Miralax because I associate it with a bout of cramps a while back. the doctor says to take benefiber but I am scared because I don't want to have these symptoms. right now for the last two days I have only eaten eggs and have moved over too a mostly protein diet thinking maybe I have SIBO. scared to eat anything else.

I've had IBS-D for over twenty years so I truly feel your pain. I had one bout of constipation back in September 2012 that started this nightmare for me. I'm 8 days post LIS and tag removal. I've taken Benefiber for years and it really helps me. The trick is finding the right amount for you. I take very little. 1 1/2tsps per day in the morning and I always take probiotics too. For me that really helps. Also know your food triggers. Lettuce kills me! The diarrhea feels so much worse on the fissure for me due to it being so acidic. Praying you find relief soon!

which probiotic do you find the most helpful. I have been on align. its very expensive and does not seem to do anything
