Confused about next step...

Hi there,

i got a fissure in 2011.

My CRS never saw the fissure but guessed i had a hidden one and gave me creams, etc. ... those gave me headaches.
I tried Botox but it didn't work. After one time, she said if it didn't work, it wouldn't work.

The fissure eventually got less painful but never fully did the pain go away. Eventually the CRS referred me for pelvic floor dysfunction. No one on my insurance but private sessions were $250 or more an hour so I never went. Things settled down after a long time, "pretty well", though not so that i was perfectly pain free sitting for long periods of time.

Anyway, i went on vacation recently for the first time since then and with the plane and hotel room i had a hard dry bowel movement and excruciating pain. I went to see a CRS in the vacation city who said he couldn't see a fissure and that i must have proctitis. Date of this new problem is April 3, 2014

Well, all my symptoms are much more of a fissure than that so i discontinued the cortisone. No bleeding at all.

Now i get this burning pain after a bm that can last for hours. Ice usually feels better than heat though i am "training" myself to tolerate the heat if possible, i think it's better for me. Sometimes though it inflames things. Sitz baths help at least for a bit.

Anyway: I am scared to see a CRS. I don't want another exam. Afraid of a re-tear. I can't take the creams - they give me headaches. Maybe Botox? if i did it more than once? I'd have to see a different CRS of course.

I am just not sure what to do. Or even if i have a fissure or Levator Ani Syndrome.

At what point does one revisit a CRS for this type of thing? It feels like i went so many times the first time around.

i don't think i want surgery but i can't imagine going on living like this. I have located a pelvic floor specialist (i think) near me and did get a referral from my GP so that's good.

I can't offer you much advice on what the next steps for you are but just wanted to let you know that you are alone in your problem; I am a person who is healed and still have that burning/ stinging sensations after BM and it lingers. I am also totally confused as to what to do next. I have seen 4 doctors already and I am going back for to see two more; I am also doing physical therapy which helps but has not fully helped me get rid of this pain completely. I've read other stories of people who have had two injections of botox which helped.

3/14 Diagnosed w/ fissure given Nifedipine
4/14 Referred to Pelvic Floor Physical Therapy=Pelvic Floor Dysfunction
5/14 Fissure declared "healed"/chronic anal pain persists
9/5/14 Botox to pelvic floor
9/22 biofeedback

I would make an appointment to see a CRS. Is the pain the same now as it was in 2011. The pain you are now describing are classic fissure symptoms. Maybe it wasn't a fissure in 2011. Levator ani spasm can be caused by having a fissure, that is what I had. I had pain from my tailbone to testi, in addition to the burning. It would last for 6 plus hours after a BM. The cream you used sounds like nitro, I tried it at first and got bad headaches also, but got used to it over time. There is also another cream called nifedipine that doesn't cause headaches, it didn't work for me either. I'm now on day 41 of Botox and it's working for me. If the creams don't work most people usually do Botox or LIS. You should probably see a CRS and talk of your options.
