Don't lose heart. It can be a lengthy process.
I had to change my diet completely in order to get consistently soft bm's and only have 1 per day. I was a veggie that ate loads of fruit and veg.

Now I don't eat fibre. No fruit or veg. I have croissants, pain au chocolate, white bread, cheese, lemon cake, ice cream, cornflakes, boiled sweets. Basically low residue food (on advice of hospital dietician)

I drink at least 5 pints of non carbonated fluid per day

I take 1 sachet of Laxido per day and use GTN cream.

My pain is about a 1 on a scale of 1 to 10. At the end if Septber I am booked in for Botox as I have had fissure since Sept 2013.

you are not completely healed? are you?

Also How does all veggie and Fruit diet (fiber) helps. I tired to add more fiber to my diet and it started hurting even more because of larger stools. now days, i have more powdery BM. it helps but seems like it re-tears when i have regular/large BM. Not sure if i should stick with High fiber diet.

any advice.

H Sandman,

The purpose of fiber is so the stool does not get compacted and hard. Fiber will for most people help keep the stools soft. Your goal should be a soft formed stool on the small side with texture much like tooth paste. If you change your diet, make small changes at a time, so you can evaluate how the food change impacted your stools. Diet is different for everyone, and adjust until you get a diet that will work for you. Google the Bristol stool chart. Most people will do better with #4 and #5 stools, and some will do better on #5 and #6. At BM time your goal should be to target a BM stool size and texture that has no pain. I find that I do better with two BM's a day as the stools do not get as large and the stools are softer, but some days that is not possible. Most people are better with one BM a day. Typically the longer the stools stay in the digestive system the harder the stools become. What type of stool softener are you using? I do not see anything about water intake in your posts. You need a minimum of 64 oz of water a day and some people will go up to a 100 oz of water a day. Try to get in 2 or 3 warm sitz or a bath in ankle deep to get blood flow to the fissure. It can take the Nifediphine three to four weeks before it will relax the sphincter muscle and make it easier to pass a stool.

Hi sandman, don't worry that you are complaining too much. We all need to vent and share our suffering with people who know what it's like.

It sounds as though the nifedipine and stool softeners were helpful, but you are not healing rapidly. Sometimes it does take a very long time, unfortunately. As in, months. But it won't be so terrible all the time. As you heal, the bad days will get milder and less frequent.

And sometimes it just doesn't heal without surgery. I advise going back to your doctor and asking to be referred to a colorectal surgeon. Ideally you would get manometry done to see if you have high anal resting pressure. If you do, they may recommend LIS. Not all CRS's do manometry (mine doesn't), but anyway, go and see a specialist and get advice.

Good luck! And complain whenever you want to!
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

No, unfortunately I am not fully healed.

A diet with lots of fibre is usually recommended but I found it gave me large bm's.

No matter if these were soft the size still caused me lots of pain. I saw a dietician as I lost loads of weight due to not wanting to eat ( was 14 stone in April 2013 and am 5st 12lbs now) The dietician listened to my problem and looked at my food and pain diary.

She said that fibre is not the answer for everyone and suggested I follow a low residue & low fibre diet. Low residue means that there is not a lot left to come out in a bm. Your body uses almost all of the food. This means I eat mainly biscuits/cakes/white grain cereal and boiled sweets. If I wasn't vegetarian the dietician would have included fish in my diet.

I can eat eggs, cheese, rice as well but restrict these to make sure I only have 1 bm a day.

This is not particarly healthy as diets go and I get regular blood test. I only do it as the alternative was being too afraid to eat and lots of unpredictable bm's.

I now have control over it, but I must take the Laxido daily as well.

Hope that clarifies things. If you google low residue diet you will see you can eat quite a wide range of food. I am just sticking to what I know works for me :)

Oh yes, sits baths, lots of sitz baths help

Update-

All
I am feeling little better for few days but it feels like half of the day, i feel (mid) 4-5/10 sore specially when walking Please notice at work.However if i am home i dont feel much only while out and doing some activity etc.. Is this normal? I dont feel the stabbing pain but sometimes feel little burning on scale 3-5/10. I am going to work and driving really sucks but not as bad it used to be. Anyone experience soreness down there specially walking and in the process of healing.

Again sorry all for complaining. :needguh:

I don't get pain walking, most of my pain comes when I sit down.
