Normal day "back then", when my fissure was new:
Wake up, blissfully pain free. Lie there savouring the peace in my backside. Get up, have painful BM, be in agony for hours. Walk painfully to work, and deal with the pain by taking handfuls of ibuprofen and sitting on a hot water bottle (yes, even at work!)
By midday things had calmed down back there. If I didn't have another BM, I'd be OK-ish for the rest of the day and night. If I did, chances were I'd spend the first hours of the night unable to sleep with pain.
Next day, wake up, blissfully pain free. Repeat.

Now:
Wake up blissfully pain free. Most days, stay pain free. Some days I have a problem BM, and have some discomfort bordering on pain. Take ibuprofen, sit on a hot water bottle. Deal with the discomfort, knowing it will only be a day or three.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!
