Anal Fissurectomy + Hemorrhoidectomy

This all started when I began having pain during a bm a year ago. It was not an awful pain and I used typical over the counter hemorrhoid medicine to keep the pain at bay. At this time I was experiencing constipation. However in July my family was leaving on a big trip to Europe and I did not want this pain and constipation to accompany me!

I returned to my family doctor claiming I had severe hemorrhoids with blood on every bm (sometimes on the tissue, other times in the water.) I was aware how unglamorous this doctors visit would be and I was already dreading looking her in the eyes to explain a pain when I poop. Being a 20 year old female, not overweight but still about 175, I guess it was not common for her to see a patient like me with this issue. The nurse set up the lube and clear tube which I soon found to be extremely painful upon insertion. My doctor claimed she did not see any hemorrhoids but just a little inflammation. With that diagnosis I was off to the drug store with a prescription for Linzesse (basically an extreme laxative) and advice to use hemorrhoid suppositories which I had already tried before. The Linzesse eased the strain but I was still feeling a sharp pain when having a bm. Europe was fun but for the entire 2.5 weeks we were there, I did not have a single bm. I remember actually taking 2 Linzesse and not experiencing anything.

Fast forward to October. This pain has gone from a 7 to a 10 and I can't stand it any more. It felt like a knife was constantly being shoved up there and I also had a small swollen spot on the outside of the anus that was where the pain was usually occurring. The constipation was gone but that was worse because I became fearful of having a bowel movement. I once again went to my friend google to find a diagnosis for my painful issue. Especially after finding the swelling, I became pretty worried. My symptoms matched those of a chronic anal fissure and I took that diagnosis to my doctor promptly. She came into the room, and without examining me claimed there was no more she could do except send me to a specialist. I find this frustrating because she most likely didn't want to look at my anus again (I know its not fun) or she was not sure what a chronic anal fissure was.

I suspect the latter because once I met with a rectal surgeon and told him what I suspected it was, he examined me and concluded it was indeed a fissure. Not only that, but it was covered in scar tissue. And it had been so long, he could give me an ointment, but he overall suggested a surgery to remove it all so the problem would be gone for good. This forum and the rest of the internet already prepared me for a possible surgery so I scheduled it for a week and a half later. (Can you tell I was eager to get it gone ASAP)

Surgery day comes along, I had been taking my stool softeners and fiber, and the night before did my suppositories to rid the rectal area of any stool. Waking up from surgery was not so fun. The second I woke up I was crying hysterically (from the anesthesia they say) but I strongly believe it was from the extreme pain I was in! The surgeon came up to me and held my hand and told me they had to do more than expected but I was seconds from waking up and only wanted the pain to disappear. Eventually the nurse called my dad to the recovery area and he held my hand while they began giving me small doses of pain meds. I really didn't think it would hurt so bad! part of the discomfort came from the padding they insert into the area and it is removed in the recovery room. Thank god when she did that!

Later in the recovery room I discovered I had TWO fissures that were removed and they also found TWO internal hemorrhoids. (I told you Dr. Barber!) Apparently the pain is from the hemorrhoids being removed. My problem is I am taking the stool softeners and fiber twice a day, I had the surgery Thursday, and it is now Saturday. I feel the need to have a bowel movement but I can't. I know part of it is because I am scared of the pain but I also feel as if the hole is closed and I am pushing nothing but gas out. Also, it hurts to get up and down and I can only sit a certain way. There was something hanging out of my anus that I could feel and I thought they left some of the cotton there. Has anyone else experienced this? is it normal or did she leave padding in there? Today I had enough and tried to pull it out and a good 3 inch piece of gauze came out (very dirty with blood) and I wonder if there is more left in there or if my anus is too swollen to have a bm.

Has anyone else experienced a surgery with both removed like I have? I cannot find anything that can relate to what I had done and I would like a little bit more information on when I can expect a bm, the padding inside, and when the stiches/staples will dissolve.

Having a BM after a general anesthetic is always a bit difficult, as the anesthesia slows things down in the bowel. Having both those surgeries at once is hard on the system in general. They do sometimes pack the wound and managing to get it out is at least part of the problem. Do you have a Sitz Bath? If not, I would get one at Rite Aid, (or sit in a bath). This should help to relieve some of the pain. Poop in it if necessary, then have a shower, etc. Anything to relieve the pain. Drink water and maybe take a laxative - Milk of Magnesia is what I take.

Here are some instructions that I found too for post-surgery:

"Constipation

It is common to experience some constipation after surgery. The use of anesthetics during surgery and prescribed pain medication after surgery may lead to increased constipation and production of hard bowel movements. Often patients will state that they are “constipated” because they have not had a bowel movement within 1-2 days following surgery. Some patients experience their first bowel movement up to 3-4 days following surgery. Constipation is most often noted by symptoms of bloating, the sensation of feeling full, or straining with hard bowel movements. Usually, constipation can be relieved within 24 hours with the use of a fiber product, mineral oil, and increase in liquid intake as directed in the post operative instructions. If after following these instructions over a 24 hour period, a patient is still experiencing constipation, he/she should contact his surgeon."

By the way,having had hemorrhoid surgery myself I know just how bloody painful it is. One of the most painful surgeries you can have and definitely worse than childbirth. I wish that you will soon be out of pain an it will soon just be a memory.

Try to get off the heavy pain relievers and just go with Tylenol or anything like that.

Also, about the Sitz bath. We had a nurse on here some time ago and she too used her Sitz bath as I suggested.

I will go get a Sitz Bath if it will help! Thank you for the advice. I feel hopeful now that there is a light at the end of the tunnel!
