hello lovely people,

i'm posting this because its my 5th day of ANAL CRAMP/SPASM, ever since i got the spasm on the first day its been ongoing till now, i'm really worried every morning when having a bowel movement it just feels like someone stuff a golf into my anus and its the same feeling ever since. will this ever stop?

1. i'm interested in finding out from you guys, does this spasm/cramps occur everyday for those who had experienced it before? D

2. Does it occur during BM or after a BM?

3. What your doc recommend you to do after you told them this condition?

i'm suspecting this problem is called "levator ani syndrome"? however i don't experienced sharp pain or pain during the spasm.

Answers:

1. Yes

2. After, for hours.

3. Use Diltiazem, Nifedipine or Rectiv ointments (google them). If these do not work, then surgery, if recommended by your doctor.

The above are symptoms of an Anal Fissure. They are not symptoms of Levator Ani Syndrome. LAS has symptoms such as intermittent rectal pains that wake you from your sleep, among other symptoms.

Thank you so much Savaici for your reply.

However i used to have a fissure on the anus externally a year ago and it healed up however i don't get cramps as bad as this, i have never felt spasm like this before. on the outside it seems like there isn't and fissure. is it possible for it to happen inside, but whenever the cramps occur i don't feel any pain.

Could it be me doing too much kegals excercise recommended by my doc due to my suspected internal rectal prolapse? however no proper diagnose was made as i failed the defecography X ray as i'm unable to pass out the barium paste. i realized i would always focus on tightening and closing the anus previous while doing kegals and definitely got it all wrong and messed up. Could this very likely be one of the reasons?

Are you seeing a CRS? I really do think that you need to chat with your specialist and see what he suggests. I had trouble with kegel exercise years ago, so not done them since. Anything to do with a suspected prolapse (but it has not been diagnosed?)we are not really experienced enough on here to advise you about, but really sorry you are having to go through all this.

really appreciate the information.
yeap basically i failed one of the proctography test as i'm unable to released the barium they put into my bum due to my poor coordination of the pelvic floor muscles which i believe it should some sort of pelvic floor dysfunction.

however what worries is me is the sudden cramp and spasm which i had never experience before :( i also took the anal mamoetery test which shows i have a high resting pressure.

The cramps are usually during my BM, my anus literally felt like someone put a golf ball in there, and it lasted like 20 mins however with no pain. when i touch my anus/sphincters muscles,they were all so stiff and tight, just like how we would feel when we flex our biceps. my appointment is on the 13 january, mean while i just gotta find out more from everyone here :(

Do you literally feel the anus muscles stiffness and contracting when it spasm?
because my symptoms some how matches levator ani syndrome, however i don't experience any pain, its only the golf ball feeling in my anus every morning during a BM

With an AF, the pain is similar to a cramp you wake from in the night, in the calf of your leg, except that it goes on for hours. It is the spasming of the involuntary internal sphincter. The pains are worse than being in labour, in my opinion... :affraid: :affraid:

Anyway, you should get some answers from your appointment on the 13th, and I hope you are seeing a very experienced specialist! :sunny:

yes, i also get that involuntary spasming of the internal sphincter, however i don't experience any sharp or trobbing pain like what many of you had mention hear after reading through those post. its more of a very tight and the feeling of siting on a golf ball during BM, afterwards and throughout the day. my anus/rectal just feel so tight and clench up.

i tried everything on the net and what people had suggested here but nothing works:
-Sitz bath
-magnesium supplements (even tho at half dosage i still have loose stools.
-massaging externally with my fingers
-doing Diagrammatic breathing (it used to work before i had this spasm/cramps, but not anymore as i can't relax my muscles now when its in the constant state of cramp/spasm.)

However i can't excess to any of the muscle relaxant meds whether taken orally or in the form of ointment such as glycerin trinitrate. (MOST OF IT ARE CONTROLLED DRUGS HERE IN SINGAPORE)

hence i could only hope my specialist would write me a prescription for some muscle relaxant on 13th jan.

MEANwhile, is there anything that i haven tried that could be recommended by you guys? :(
i'm really going crazy,i'm only 21 and i have to go through this. i really think that i might have depression, i have suicidal thought because of all this problems for the past year.

Believe me, I know the depression this can bring on. I lost 40lbs in weight... I was also depressed, but was lucky in that I did not have to go out to work, etc. If you feel really bad please talk to someone about it, or see your GP soon. You should not have to suffer. 

None of the meds for this are available without a prescription here in the USA. They (Diltiazem, Nifedipine and Rectiv (nitro) ) are not muscle relaxants - not benzodiazepines or anything like that. Here is a description of what they are and do.

Externally applied nitroglycerin (Rectiv), to help increase blood flow to the fissure and promote healing and to help relax the anal sphincter. Nitroglycerin is generally considered the medical treatment of choice when other conservative measures fail. Side effects may include headache, which can be severe, but you can get used to this gradually. 

Topical anesthetic creams such as lidocaine hydrochloride (Xylocaine) may be helpful for pain relief. Do see if you can get something. 

Blood pressure medications, such as oral nifedipine (Procardia) or diltiazem (Cardizem) can help relax the anal sphincter. These medications may be taken by mouth or applied externally and may be used when nitroglycerin is not effective or causes significant side effects. Here in the US they have to e compounded by a pharmacy.

I did and do take a very low dosage of benzodiazepine to sleep (which helped me incredibly when I was in constant pain daily). You might ask your doctor if he would give you a low dose muscle relaxant/anxiety medication to help.

Sigh, i just gotta wait for my appointment and see what the doc would suggest. It really annoys me when it cramp up every morning during a BM, i couldn't even relax it properly for stools to come out. if wasn't for the stool softeners and magnesium, i would be tearing it anytime soon. I'm so desperate for help, i tried everything i could but to no relieve.

however i'm glad to know, i'm not the only one suffering from all these. i just hope everything can be over soon :(

i read up online about anal dilation and realized this method isnt used anymore. i wonder will it work for my case to open up the sphincter. have you had any experience with this? There isn't any anal dilation here in Singapore though.

Hi there ML!

I did dilation, and on this link you will find the information which I've given to others about what to do. One of our Moderators, Suzyljank (who had double-flap surgery), still dilates on the advice of her surgeon. Another person was wecc, who had Controlled Anal Dilation (also known as Standardized Anal Dilation) in New York, and it is also done in some of the teaching hospitals here, including the Cleveland Clinic (just incase your doctor says that dilation is no longer done,we are not talking Lord's Dilation here).
There is a lot on that whole thread which is just for information as it is now locked. One of the links on the post you might have to copy and paste.
