Worst pain of my life, please help. :(

Hi everyone,

I would just like to talk to someone about what I'm going through. I was DX with hemmrhoids on Sept 18 due to a gradual building cramping pain after I move my bowels, have been in literal pain ever since. My general doc examined me, said she did not see fissures but I have all the symptoms. About 4-10 min after I go, I get a slow burn and cramp that builds into a full blown spasm. I can't breathe, walk, talk let alone think. I have had bits of bright red blood in the past and one Lima bean sized hem I had after giving birth but it never gave me this problem. My doc won't refer me to a specialist i would like to see because it's out if the network and my insurance may not cover it, but I need some relief. Things I've tried: hydrocortisone 2.5%. sitz baths 4 times daily, slight increase in fiber. I don't know what to do and the pain is with me all day until I fall asleep. :((((Thanks for listening.

Sorry to hear about your pain. I think it is possible to have cramps without fissure if muscles are too tight, but it is less likely. Sometimes fissure can be difficult to see during examination. Have you tried nitro or diltiazem ointments? Hope things get better.

Thank you, no, my doctor won't prescribe it for some reason. Going to try to get into see a gastroenterologist this morning. These pains have me literally falling over and they last about 2-3 hours and then I'm left with stinging and burning all day. :( Is this a normal presentation of a fissure?

Does anyone have any recommendations to stop the cramping?? It doesn't happen right after I go, but it slowly builds until all I can do is lay down and cry, take ibuprofen abd wait until it passes. I tried this morning to jump right into a hot bath but it didn't stop it from building. I so scared, in so much pain.

Are you cramping in the stomach or in your anus/rectum, could you try and see a CRS.
I was examined by 3 different docs and it wasn't till I saw my CRS that I was diagnosed with my very large deep fissure...so even fissure you would think are noticeable can be missed by general docs ( mine was hidden behind a large hem).
For the moment I would stop the steroid cream as it can thin the skin with long use which isn't great for fissures.
You need a diagnosis as quick as possible as there are lots of things that can at least minimise the agony of a spasm if not totally stop them.
Try to get to the CRS. Until then try and have the sitz bath as hot as you can to help reduce the spasms.
I can't take ibuprofen as I have a very sensitive stomach, but I do rub the ibuprofen gel on my butt cheeks and lower back and this does help a bit.
My pain never started until 20/30 mins after a BM and then crescendoed into agony after about 2 hours and lasted for anything up to 10 so you don't have to feel pain during a BM to have a fissure

Thanks. The cramping is right in the "ring" of my anus...I feel ok when I go but sure enough 4-10 after I'm laying on the floor biting a sock!!! That cramp lasts 1-1.3 hours, then I'm just burning all day long. Occasionally I'll have jabs and stabs! I'm working on trying to see a gastroenterologist, they cannot see me until Nov 9th!! So I made another appointment, 4th one to my GP for tomorrow. She just thinks I'm constipated, which I'm not, normal morning movement, usually pretty soft... Thinking the emergency room might be the best bet! I am so afraid if going to the bathroom tomorrow!! I just need one method to stop the onset of cramping. I rest can't go through that again in the morning. :(

You might be better asking to see a colorectal surgeon as they deal more with fissures.
Are you in the US because if your in the UK don't bother going to the emergency room the doctors there are no specialist and prob won't see your fissure.
There is nothing that stopped my cramping until I got the GTN cream and that really is my life saver..some people say Valium works but it didn't do much for me....I had Botox which was ok but the fissure is still there and the pain is back now the Botox has worn off. I'm having LIS soon just got to wait for a date.
I'm sure once you get a proper DX then you get the cream you will so much better
Oh yes I know the rolling on the floor agony of the spasms

Yes, I am in the US. My husband is from the UK he keeps telling me we should move back there.
I do know the process here with referrals and long wait times is equally excruciating. Our ER services here are not terrible but from what I have been told there are long wait times. I'm so sorry you know what this feels like but in ways happy to not be alone. Your surgery from what I read is the curative. I'll let you know what they say to me tomorrow. X

Hope it went well and that you managed to get a DX if it is a fissure I really hope they have given you some cream and have a plan in place for you. The waiting times here are horrendous I should have had my LIS in September but I have only just had my prep op assessment so LIS could take another 5 weeks.
