Okay, so I had a long post but lost it due to computer malfunction (okay, maybe it was user error) so I'm going to try and be more direct this time.


Background: 30yo male, has been dealing with something (more on that in a second) for two years.


I'm not sure if I have a fissure, hemorrhoids, both or something else entirely. I don't have health insurance but I have seen one specialist and then when I had health insurance for a small time, one GP and another specialist.
Basically, my anus seems fine until a BM, then 1 - 3 inflamed "bulbs" come out of it. They are extremely painful and extremely sensitive to the touch. The two specialists said I have a fissure, but they never saw the bulbs, only the GP saw the bulbs, and she diagnosed them as hemorrhoids. The specialists (you'd think) would know better, but they also didn't seem that interested in me and seemed more interested in getting me out as soon as possible. Regardless, they both diagnosed me with an Anal Fissure, but again, they never saw the bulbs when they were active. I know that bulbs can come out of fissures, but these are larger and more numerous than has been described to me.


In any case, it could be a fissure, 'roids or both. All I know is that I once ended up on here when I was desperate for help, so maybe someone with the same condition will as well. Besides, maybe someone here who has experienced the same thing can tell me which doctor to believe.

How bad was it? Well, they would bleed probably 50% of the time during or after a BM. Often through my underwear and pants, causing a big red circle on my ass. This obviously made going out in public or even going to work a horrible endeavor. I often held in my BMs through eight hours of work, plus an hour of travel each way, out of the fear that I could have an embarrassing episode at work. I know that is bad for AFs and 'roids, but the other options were either calling out or having my coworkers (plus clients etc) see me with a big red circle on my ass.

Besides the embarrassment, the pain was unbearable. My method at the time was to just go, apply cream and then lie down until it went away. This would take between 1-4 hours, usually closer to 4 than to 1. Obviously that is no way to live, with ~half of my free time taken up either passing a BM or recovering from one.

My method hasn't healed these bulbs, but it has made my life much MUCH better, to the point where I am no longer considering surgery because the pain is short and manageable. With this method you can use any cream (I started it with a prescription cream but changed to prep H when that ran out and have not noticed any difference).

Basically, when taking a BM, do all the normal rules: Don't strain and be nice to your anus. I like to use a squatty potty and flushable wipes at this stage. (I know they dry out your anus, but we will be hydrating it soon). Once it is as clean as you can get it without harming the bulb, you then move to the next step.

Take some TP and fold it a few times and wet it with as hot as water as you can get from the bathroom faucet (or wherever). Then carefully clean your anus. Next, take another set of TP, wet it again, stick it onto the bulb and stand up, squeezing the wet TP between your butt cheeks. Make sure to stand directly above the bowel because some water is going to drip out and you don't want that to fall into your pants. Squeeze a few times as hard as you can. If you like, this step can be repeated twice for better results. I think this reduces the inflammation and numbs it a bit for the next step (but I'm not a doctor, all I know for sure is that it feels good and I have much better results when I do this)

Then, take out the TP and flush it, pat dry your anus (and the bulb) as carefully as possible. Then put on your medicine/cream of choice on the bulb. Take some TP and fold it as before but this time DO NOT wet it. Take the dry TP, put it on the bulb (which now has cream on it) and pull up your pants. Then, put the toilet seat cover down, and sit on it. You can sit on any hard surface, but there is more work to be done so you might as well stay in the bathroom. The dry TP has several uses: It stops any blood or liquid from staining your pants, puts pressure on the bulb (very important, and I think the key to the whole thing) and it provides some cushion as opposed to hard plastic or wood.

Adjust your weight so you feel some pressure (but either no pain, or a minimal amount of pain) on the bulb and wait 5-10 minutes. Oftentimes, I can actually feel the bulb(s) retract back into my anus. This feels really weird, almost like a BM in reverse. Sometimes though, I don't feel anything at all. In either case, the bulbs are usually gone within five minutes. A few times this method hasn't worked, but what I did then was remove the dry TP, reapply the cream and put new TP in between my cheeks and try sitting down again. I haven't had it not work on a second attempt yet.
Anyway, after the bulbs go away, flush the TP, CAREFULLY wipe your anus one more time (to get rid of excessive cream and to get any crap that was hiding under/alongside the bulb). Flush everything, and walk out of the bathroom feeling like a new man (or woman)!

One note: Soft seats don't seem to help. You have to sit on something relatively hard, so that pressure is put on the bulbs. My toilet cover works perfect, as it is hard but indents a little when I sit on it, providing some give. I have sat on wooden chairs during the last step before, and that worked just as well, but was a little more painful during the process. FYI: The TP actually makes the pain less than what it would be if I just sat down on a hard surface (which used to be a nightmare after a BM) but Cushioned seats (including my computer chair) have proven to be no help at all (for me at least)

Also, this method gets rid of the bulbs so quickly, I can actually feel relieved after taking a BM. Remember that endorphin-enabled high you would sometimes feel after a large BM before AF or hemorrhoids ruined it? Well, I actually get that feeling again! I honestly forgot how good it can feel to take a crap! I can be out of the bathroom five minutes after finishing a BM with no pain at all.

Compared to feeling miserable for four hours afterwards, I feel healed by comparison (though, I have some work to do with my diet etc. because eventually I want to be rid of the bulbs all together).

So, it is not necessarily a "cure" but it is a success story, and hopefully it will help someone else here.

Also, wanted to add that I've been using this method for about 6 months now. And while they still exist, the bulbs seem to be getting smaller with time. (I had a setback when I stupidly ate an entire pizza to myself and then had an epic BM that reopened them. Besides that set back, which was entirely due to my stupidity and over-confidence, things seem to be getting better everyday)
