Hi, I am a 30y/o female new to this forum. I am currently dealing with a chronic anal fissure that has been going on now for 5 months. It came on as a result of constipation and stress and was misdiagnosed as hemorrhoids for a few months. This has been a nightmare! Everytime I think I get some relief I end up back at square one. What I'm currently doing: nifedipine cream 3x/day, taking supplements from gutsense.org which really seem to help, eating 30g of fiber per day (all plant based fiber no grains), drinking a lot of water, acupuncture, and i started pelvic floor physical therapy. I have also had issues with tight vaginal muscles/painful sex in the past so PT might help with that too. Oh and I started taking zoloft for all the anxiety this is causing me. Can anyone share if it seems like I am doing the right things or if you have any tips for what else I can be doing? I am getting so desperate and don't want to have to have surgery.

Hi there and welcome to the forum. So sorry that you're suffering with this. Have you seen a colorectal specialist for your fissure? They're the best ones to deal with this. You sound like you're doing the right things, but may need more intervention at this stage. Botox is one option you can try, and since it's already been five months, I recommend getting an opinion on it soon if you're inclined to do it. Warm sitz baths can help a lot with soothing pain and tension in the area as well. I hope you get some relief from the physical therapy as well!
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
