Pregnant and in Pain

Hi all,

I need help and at my wits end. This pain is unbearable! I'm 5.5 mos pregnant and a month ago was diagnosed with an anal fissure. Holy moley! The pain is intense - it gets better and then wamo, I can't sit for 6 hours.

I've been plant based for 8 mos and I have 1-3 stools a day all loose or extremely soft. I have had 2 kids without a problem and feel that the pressure to defecate has caused this.

The dr performed an anoscope and I about died. That was beyond horrible and I will not return to do that again. I've been given a Dilitazem compounded cream to use with a question as to it being ok for the baby. After much research I use a small amount 2 - 3 times a day. Also tucks, moist wipes, sitz and lidocaine - but that seems completely ineffective. Loose stools are the worst to pass in my case. I don't really know why, perhaps they are more acidic..?

Any ideas or help is much appreciated. The drs seem uneducated especially with being pregnant and almost everything can't be done while pregnant. Let alone the thought of pushing out a babe is unbearable.

Try to heal fissure by yourself before the birth of the child.. I am with fissure 5 Years, now it’s open, it’s hard to manage with this shit, I think that you need another doctor, you need a team because you are pregnant and Birth will be more painful because of the fissure.. do you have bleeding from fissure? Is rip of fissure large? For pain for me is best Cold Cold water, after this I use ultraproct L Cream, warm baths for relaxing anus muscle, I have tried to relax the anus with my fingers, like a massage, not inside the anus, next to anus side, is very helpful .

Thank you for replying. I just emailed my Dr and asked for a better referral for out of the area. There has to be someone experienced with this and pregnancy. I can't imagine giving birth - let alone the aftermath. I do not have any bleeding. I imagine it's large but the Dr didn't say. I wonder if salt water would help? I'll have to look up the medicine you recommended!

Hi,
Make sure you use your cream and have sitz baths as often as possible, but please try and get your BMs no looser than ice cream as loose stools can be more damaging than hard ones for a fissure, as they are acidic and can strip the skin which as you can imagine is not ideal for a cut. Also loose BMs will leave more residue and thus produce more pain.
I got my fissure due to crohns (extremely loose BMs) and it went from a couple of centimetres to two inches and down to the muscle within 6 months all due to acidic loose BMs
I would recommend putting on a barrier cream nappy cream is ideal before a BM to protect the area if your BM is loose.
I cannot imagine going through a fissure whilst pregnant, but hopefully the diliatazem will help and barrier cream will help too.

Wow thanks for the great info!

Marko BM is bowel movement.

Makes so much sense because the loose stools are worse than any others I've had. How are you healing yours? I can't imagine down to the muscle.. sorry to be graphic but you put the barrier cream inside? I honestly haven't been able to find the exact spot of the fissure. It feels like it's right there but as soon as I try to find it I can not. It's probably why Lidocaine isn't working.. and maybe the Dilitazem would work better if I can get it there..

Mine healed through LIS I tried creams and Botox as no one wanted to do LIS due to my crohns but I researched and found a CRS that specialised in IBDs and he was honest said if I had a flare during healing it would be hell and that even after LIS if I had a prolonged flare I could get another fissure...which I now have...but I would happily have LIS again
Yes put the barrier cream inside you don’t really need to know exactly where the fissure is just coat the whole area. I never found my fissure until after Botox when my IS had relaxed enough. Three CRSs couldn’t see my fissure as it started behind a hem and then went up inside the anal canal it was only my current CRS who saw it and that was without an anoscope, he just moved my hems and was like “ah I thought it was probably hiding” lol
On a side note if you have loose BMs then perhaps if possible try and clean with the shower head after as that is the best way to get rid off any residue even wipes can leave a bit behind and the wiping motion can aggravate the fissure

I know what is bowel movment, word loose, that i don’t know :))..today in morning i Had stool, it was Soft :), after that i wash my anus with Cold water(i preferred Cold water ) than i put little ultraproct L cream on finger and easy put in my anus just a half inch even less and than I put diltiazem around anus(not inside)than with finger I am massaging sphincter( it works because it relax the muscle, I had two months hard large stool and nothing happened, and I didn’t mind on what I am eating, but last week I was exaggerating with chocolate and junky food and Bm was extra large and fissure exploded ) sorry if my English is bad, I hope so that you understand me :)

Hi Marko loose is a BM that isn’t formed or is liquid with a few formed bits in there
Anything on the Bristol stool chart that is above a 4,
If you don’t know the Bristol stool chart just google it and the images will show you much easier than me trying to describe it
